[目的]为有效预防子痫前期的进展,了解血栓前状态诊断指标在子痫前期发生发展规律。[方法]选择2009年1月~2010年12月收住的52例子痫前期患者,其中子痫前期轻度者28例,子痫前期重度者24例,另选择健康体检的孕妇32例作为对照组。[结果]正常孕晚期组PT、APTT及PLT各平均值与子痫前期轻度组对照,差异无统计学意义(P﹥0.05);而AT-Ⅲ平均值显著高于子痫前期轻度组(P=0.022﹤0.05),DD平均值显著低于子痫前期轻度组(P=0.018﹤0.05)。子痫前期轻度组PT和APTT各平均值与子痫前期重度组对照,差异无统计学意义(P﹥0.05);而PLT、AT-Ⅲ平均值显著高于子痫前期重度组(P﹤0.05),DD平均值显著低于子痫前期重度组(P=0.011﹤0.05)。[结论]AT-Ⅲ、PLT及DD是提示血栓前状态的敏感因子,妊娠期血液AT-Ⅲ和PLT水平呈现下降,而DD水平呈现上升趋势时,提示妊娠高血压患者血栓前状态较为严重,子痫前期正在进展。
[Objective] In order to effectively prevent the progression of preeclampsia and to understand the development of prethrombosis state in preeclampsia. [Method] Fifty-two patients with preeclampsia admitted from January 2009 to December 2010 were selected, including 28 with mild preeclampsia, 24 with severe preeclampsia, and 32 with pregnant women selected as healthy controls group. [Results] The average PT, APTT and PLT in normal pregnancy group were not significantly different from those in mild preeclampsia group (P> 0.05), but the mean values of AT-Ⅲ were significantly higher than those in mild preeclampsia group (P = 0.022 <0.05). The mean of DD was significantly lower than that of mild preeclampsia (P = 0.018 <0.05). The mean of PT and APTT in mild preeclampsia group was not significantly different from those in severe preeclampsia group (P> 0.05), while the average values of PLT and AT-Ⅲ were significantly higher than those in severe preeclampsia group (P < 0.05). The mean of DD was significantly lower than that of severe preeclampsia (P = 0.011 <0.05). [Conclusion] The AT-Ⅲ, PLT and DD are the sensitive factors of prethrombosis state. The levels of AT-Ⅲ and PLT in blood during pregnancy decrease, while the rising trend of DD shows that the prethrombotic state of pregnancy-induced hypertension is more serious. Preeclampsia is progressing.
