This article investigates the correlation between late ventricular potential after myocardial infarction and the pairing interval necessary for induction of sustained ventricular tachycardia or ventricular fibrillation. Subjects were 83 patients with previous myocardial infarction, 56 patients had a history of persistent VT episodes, 15 had episodes of VF, and 12 had unexplained syncope. Ventricular grading procedure induced 71 cases of sustained monomorphic ventricular tachycardia (SMVT) (as VT group), induced ventricular fibrillation in 12 cases (VF group). Both SMVT and VF were induced by SR-S_2-S_3 or S_1-S_2-S-3 stimulation. The sum of paired interval (SCI) required for VT and VF induced by VT was significantly longer than VF (485 ± 59 vs 387 ± 36 ms, P <0.001), and the difference between effective refractory period and second pairing interval was less than that of VF group -3 ± 40 to 24 ± 29 ms,
