目的 :探讨基质金属蛋白酶组织抑制因子 - 1(TIMP - 1)和胶原蛋白Ⅳ (ColⅣ )的定位表达与前列腺癌浸润转移的关系。方法 :采用超敏免疫组化S -P法并结合病理观察对 4 3例前列腺癌 (PCa)和 7例良性前列腺增生 (BPH)标本中的TIMP - 1及ColⅣ表达进行分析研究。结果 :TIMP - 1在PCa标本中的表达明显低于BPH标本中的表达 (P <0 .0 1) ,TIMP - 1的表达与PCa分期、分级呈负相关 (P <0 .0 1,P <0 .0 5 ) ,浸润性肿瘤明显低于局限性肿瘤 (P <0 .0 1) ,有转移者低于未转移者 (P <0 .0 5 )。BPH组织的基底膜中ColⅣ的表达高于前列腺癌 (P <0 .0 1) ,ColⅣ的表达与肿瘤分期、分级负相关 (P <0 .0 1)。TIMP - 1的表达与ColⅣ的表达存在着显著正相关性 (P <0 .0 1)。结论 :随着组织中TIMP - 1的表达下降 ,加速了BM及ECM的破坏 ,使PCa更具向周围侵袭转移的趋向。检测PCa标本中TIPM - 1和ColⅣ的表达 ,是预测PCa恶性趋向及预后的一种简便实用的方法。
Objective: To investigate the relationship between the expression of tissue inhibitor of metalloproteinase - 1 (TIMP - 1) and collagen Ⅳ (Col Ⅳ) and the invasion and metastasis of prostate cancer. Methods: The expressions of TIMP - 1 and Col Ⅳ in 43 specimens of prostate cancer (PCa) and 7 specimens of benign prostatic hyperplasia (BPH) were studied by S - P immunohistochemistry and pathological examination. Results: The expression of TIMP - 1 in PCa specimens was significantly lower than that in BPH specimens (P <0.01). TIMP - 1 expression was negatively correlated with PCa staging and grading (P <0.01, P <0.05). The infiltrative tumors were significantly lower than those of the localized tumors (P <0.01), and those with metastasis were lower than those without metastasis (P <0.05). The expression of ColⅣ in BPH was higher than that in prostate (P <0.01). The expression of ColⅣ was negatively correlated with tumor stage and grade (P <0.01). There was a significant positive correlation between TIMP - 1 expression and Col Ⅳ expression (P <0.01). CONCLUSION: With the decrease of TIMP - 1 expression in tissues, the destruction of BM and ECM is accelerated, and the tendency of PCa to invasion and metastasis to the surroundings is more. Detecting the expression of TIPM - 1 and ColⅣ in PCa specimens is a simple and practical method to predict the malignant tendency and prognosis of PCa.
