目的:观察并探讨HAG方案联合中药治疗老年急性髓系白血病(Acute myeloid leukemia,AML)的临床疗效。方法:选择自2011年5月—2014年3月在医院接受化疗的AML患者96例,按照随机分组法将96例患者分为两组,观察组和对照组各48例,对照组单用HAG化疗方案;观察组在对照组基础上联合中药治疗。统计两组患者临床疗效,治疗前后血红蛋白(Hb)、白细胞计数(WBC)、血小板计数(PLT)以及不良反应。结果:观察组缓解率为87.5%(42/48),对照组为80.0%(40/48),两组相比不具有统计学意义(P>0.05);两组治疗前Hb、WBC和PLT相比,差异不具有统计学意义(P>0.05),治疗后,观察组Hb、WBC和PLT分别为(68.7±7.5)g/L、(3.4±1.0)×109/L和(51.2±8.9)×109/L,对照组为(85.2±7.1)g/L、(5.2±1.3)×109/L和(72.4±7.2)×109/L,观察组Hb、WBC和PLT明显低于对照组(P<0.05);观察组骨髓抑制、感染、出血和肝肾毒性发生率明显低于对照组(P<0.05)。结论:HAG方案联合中药治疗AML有助于减轻化疗不良反应,提高化疗难受性,值得在临床上进一步推广。
Objective: To observe and discuss the clinical efficacy of HAG combined with traditional Chinese medicine in the treatment of Acute Myeloid Leukemia (AML). Methods: Ninety-six patients with AML who received chemotherapy in the hospital from May 2011 to March 2014 were divided into two groups according to the randomization method: 96 cases in each group and 48 cases in observation group and control group. HAG Chemotherapy; observation group on the basis of the control group combined with traditional Chinese medicine. The clinical efficacy, hemoglobin (Hb), white blood cell count (WBC), platelet count (PLT) and adverse reactions before and after treatment were calculated. Results: The remission rate was 87.5% (42/48) in the observation group and 80.0% (40/48) in the control group, with no significant difference between the two groups (P> 0.05). Hb, WBC and PLT (P <0.05). After treatment, the levels of Hb, WBC and PLT in the observation group were (68.7 ± 7.5) g / L, (3.4 ± 1.0) × 109 / L and (51.2 ± 8.9 ) In the control group were (85.2 ± 7.1) g / L, (5.2 ± 1.3) × 109 / L and (72.4 ± 7.2) × 109 / L, respectively. The Hb, WBC and PLT in the observation group were significantly lower than those in the control group (P <0.05). The incidences of myelosuppression, infection, hemorrhage and hepatorenal toxicity in the observation group were significantly lower than those in the control group (P <0.05). Conclusion: HAG regimen combined with traditional Chinese medicine treatment of AML can help reduce the adverse reactions of chemotherapy and improve the treatment of chemotherapy, which is worth further promotion in clinic.
