目的:分析送检标本中鲍曼不动杆菌的分布特征和耐药趋势,为临床合理用药提供依据。方法:回顾性调查2016年全年临床分析220株鲍曼不动杆菌的标本分布情况、主要感染的科室以及对抗菌药物的耐药状况。结果:220株鲍曼不动杆菌的主要来源是痰液,其次为伤口分泌物。ICU的检出率最高,其次为呼吸消化科。鲍曼不动杆菌对氨苄青霉素和哌拉西林耐药率已达到98.20%和78.25%;第三代、第四代头孢类头孢曲松、头孢他啶、头孢噻肟、头孢哌酮、头孢吡肟的耐药率分别为85.91%、82.24%、80.57%、12.23%、75.64%;亚胺培南和美罗培南、氨曲南耐药率为56.40%、78.51%、65.18%;庆大霉素和妥布霉素耐药率为81.46%和73.55%;左氧氟沙星和环丙沙星耐药率为58.87%和79.66%;复方新诺明耐药率为86.72%;丁胺卡那耐药率为86.72%;米诺环素耐药率最低,为10.03%。结论:鲍曼不动杆菌是医院内感染中的条件致病菌,对多种抗菌药物耐药率高和多重耐药性,呈逐年上升趋势。
OBJECTIVE: To analyze the distribution characteristics and drug resistance of Acinetobacter baumannii in the specimens for clinical examination and provide basis for rational clinical use. Methods: The clinical data of 220 specimens of Acinetobacter baumannii from clinical analysis in 2016 were retrospectively surveyed, the main infection departments and the resistance to antimicrobial agents. Results: The main source of 220 Acinetobacter Baumannii sputum, followed by wound secretions. ICU the highest detection rate, followed by respiratory digestion. Acinetobacter baumannii to ampicillin and piperacillin resistance rates have reached 98.20% and 78.25%; the third and fourth generations of cephalosporins ceftriaxone, ceftazidime, cefotaxime, cefoperazone, cefepime The resistant rate of aztreonam was 85.91%, 82.24%, 80.57%, 12.23% and 75.64%, respectively. The resistance rates of imipenem and meropenem to aztreonam were 56.40%, 78.51% and 65.18% The rates of resistance to neomycin were 81.46% and 73.55%, those of levofloxacin and ciprofloxacin were 58.87% and 79.66%, respectively. The resistance rates of cotrimoxazole and corynebacterium were 86.72% and 86.72%, respectively. Minocycline has the lowest resistance rate of 10.03%. CONCLUSION: Acinetobacter baumannii is a pathogenic bacterium in nosocomial infections with high resistance and multiple drug resistance to various antimicrobial agents, showing an increasing trend year by year.
