目的:探讨磁共振(MRI)弥散加权成像(DWI)及表观扩散系数(ADC)在脑转移瘤诊断中的价值。方法:对疑似脑转移瘤患者161例行弥散加权成像并做诊断实验研究。测定DWI对脑转移瘤的敏感性、特异性和准确性;同时与病理结果随访对照,分别测定肿瘤实质、坏死/囊变部分、瘤周水肿部分及正常脑实质ADC值,并与确诊脑胶质瘤进行比较。结果:(1)DWI对脑转移瘤诊断的敏感性、特异性、准确性分别为82.27%、91.46%和86.95%;(2)脑转移瘤肿瘤实质部分ADC值为(1.08±0.37)×10-3mm2/s,坏死/囊变部分ADC值为(1.90±0.36)×10-3mm2/s。与瘤周水肿区及正常脑实质ADC值比较,差异均有统计学意义(P<0.05)。脑转移瘤与胶质瘤不同区域ADC值比较无明显差异(P>0.05)。结论:DWI成像对脑转移瘤的诊断有较高价值;瘤灶及瘤周ADC值的测量有助于鉴别诊断。
Objective: To investigate the value of diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC) in the diagnosis of brain metastases. Methods: 161 patients with suspected brain metastases underwent diffusion-weighted imaging and do diagnostic experiments. The sensitivity, specificity and accuracy of DWI for brain metastases were determined. At the same time, the ADC values of tumor parenchyma, necrosis / cystic change, peritumoral edema and normal brain parenchyma were determined with the follow-up of pathological results. Tumors were compared. Results: (1) The sensitivity, specificity and accuracy of DWI in the diagnosis of brain metastases were 82.27%, 91.46% and 86.95% respectively. (2) The ADC value of the substantial part of brain metastases was (1.08 ± 0.37) × 10 -3mm2 / s, necrosis / cystic part of the ADC value (1.90 ± 0.36) × 10-3mm2 / s. Compared with the edema of peritumoral area and normal brain parenchyma, the differences were statistically significant (P <0.05). Brain metastases and gliomas in different regions of ADC value was no significant difference (P> 0.05). Conclusion: DWI imaging has high value for the diagnosis of brain metastases. The measurement of tumor size and peritumoral ADC value is helpful for the differential diagnosis.
