【目的】探讨肾病专用营养品在非透析慢性肾功能不全患者中对营养状态和钙磷代谢的短期治疗作用。【方法】把59例慢性肾脏病(CKD)3至4期的住院患者随机分至试验组(29例)和对照组(30例)。两组均给予个体化营养咨询,试验组另外给予口服肾病专用营养品(60 g,每日3次),干预10~30 d,检测两组干预前后营养状态、钙磷代谢和肾功能的指标。【结果】试验组和对照组分别干预(15±5)d和(15±4)d。干预后试验组血磷水平显著下降[ΔP=(-0.14±0.28)mmol/L,P=0.014]且变化量与对照组间有显著差异(P=0.038);试验组总蛋白水平无显著变化(P>0.05)、对照组显著下降(P<0.05),两组间的变化量有显著差异[试验组(-0.56±5.26)g/L;对照组(-3.69±6.12)g/L;P=0.040]。干预后两组的肾功能指标均无显著变化(P>0.05)。【结论】非透析慢性肾功能不全患者短期补充肾病专用营养品有益于改善钙磷代谢异常和蛋白消耗状态,对肾功能无明显影响。
【Objective】 To investigate the short-term therapeutic effects of nephropathy on nutritional status and metabolism of calcium and phosphorus in non-dialysis patients with chronic renal failure. 【Methods】 59 hospitalized chronic kidney disease (CKD) patients with stage 3 to 4 were randomly divided into experimental group (n = 29) and control group (n = 30). The two groups were given personalized nutrition counseling, the experimental group were given oral nephropathy nutrition (60 g, 3 times a day), intervention 10 ~ 30 d, before and after intervention to detect nutritional status, calcium and phosphorus metabolism and renal function indicators . 【Results】 The experimental group and the control group were respectively intervention (15 ± 5) d and (15 ± 4) d. After the intervention, the level of serum phosphorus in the experimental group decreased significantly [ΔP = (- 0.14 ± 0.28) mmol / L, P = 0.014], and there was a significant difference between the experimental group and the control group (P = 0.038) (P <0.05), the control group decreased significantly (P <0.05), and there was significant difference between the two groups (-0.56 ± 5.26 g / L in the trial group and -3.69 ± 6.12 g / L in the control group; P = 0.040]. There was no significant difference in renal function between two groups after intervention (P> 0.05). 【Conclusion】 Short-term renal nephropathy-specific nutritional supplements for non-dialysis chronic renal insufficiency is beneficial to improve the abnormal calcium and phosphorus metabolism and protein consumption status, and has no significant effect on renal function.
