目的 探讨超声引导穿刺活检DNA分析对肝癌疗效判断的价值。方法 肝癌患者 2 2例 ,肝内良性增生结节 6例 ,采用超声引导穿刺活检技术 ,对肝癌患者在超声介入治疗前后和肝内良性增生结节行病理活检的同时 ,将活检新鲜组织制成单细胞悬液应用流式细胞术 (FCM )行DNA检测及细胞周期比值分析 ,观察细胞增殖比值 ,以及是否出现DNA的异倍体峰、凋亡细胞峰及其百分比。结果 2 2例肝癌组织经FCM检测 ,5例( 2 2 .7% )出现异倍体峰 ;6例肝内良性增生组织均未出现异倍体。肝癌组织的S期与G2 /M期比率均显著高于良性增生结节 (P <0 .0 1)。 2 2例肝癌经超声介入治疗后DNA分析显示G1期比率上升 ,S期与G2 /M期比率下降 (P <0 .0 1) ,治疗后 5例异倍体峰消失 ,并有 12例 ( 5 4.5 % )出现凋亡细胞峰。结论 采用超声引导穿刺活检 ,不仅可提供病理形态学信息 ,还可将抽吸的组织应用FCM进行DNA分析 ,为肿瘤疗效判断与预后提供有效的生物学判断依据
Objective To explore the value of ultrasound guided biopsy DNA analysis in judging the efficacy of liver cancer. Methods Twenty-two patients with hepatocellular carcinoma and 6 intrahepatic benign nodules were treated with ultrasound-guided biopsy. The biopsy fresh tissue was made at the same time as the biopsy of liver cancer patients before and after ultrasound intervention and intrahepatic benign hypertrophy nodules. The single cell suspension was subjected to flow cytometry (FCM) for DNA detection and cell cycle ratio analysis to observe the cell proliferation ratio, and whether there was an aneuploid peak, apoptotic cell peak and percentage of DNA. Results Twenty-two cases of hepatocellular carcinoma were detected by FCM, 5 cases (22.7%) showed aneuploid peaks, and 6 cases of intrahepatic benign hyperplasia had no aneuploidy. The ratio of S phase to G2 /M phase in HCC tissues was significantly higher than that in benign nodular nodules (P < 0.01). DNA analysis of 22 cases of hepatocellular carcinoma after ultrasound intervention showed an increase in G1 phase ratio, a decrease in the ratio of S phase to G2/M phase (P < 0.01), disappearance of 5 aneuploid peaks after treatment, and 12 cases ( 5 4.5%) Apoptotic cell peaks. Conclusion Ultrasound-guided biopsy can not only provide pathological morphological information, but also use the FCM for DNA analysis to provide an effective biological judgment basis for tumor efficacy judgment and prognosis.
