目的 探讨A型超声对甲状腺相关性眼病的诊断价值。方法 分析 3 7例甲状腺相关性眼病患者 (73只眼 )有 1 9例非甲状腺相关性眼病者 (3 8只眼 )的A型超声结果 ,与临床表现对照 ,进行统计学分析。结果 A型超声诊断甲状腺相关性眼病的灵敏度为 91 78% ,特异度为 89 4 7% ;甲状腺相关性眼病患者各直肌厚度明显增加 ,与非甲状腺相关性眼病者比较 ,存在显著性差异 (均P <0 0 1 ) ;本组资料以多条直肌受累为主 ,4条直肌均受累者占 57 53 % ,1条肌肉受累者仅占 5 4 8% ;上直肌受累频率最高 (90 4 1 % ) ;各直肌厚度的平方和与该眼突出度呈高度相关 (r =0 78,P <0 0 1 ) ;9例患者 (1 8只眼 )的二次超声 (间隔 >6个月 )结果显示 ,直肌厚度呈下降趋势。结论 A型超声检查可为甲状腺相关性眼病的早期诊断提供准确的定量指标 ,并为鉴别诊断和治疗方案的选择提供重要依据。
Objective To investigate the diagnostic value of ultrasound on thyroid-associated ophthalmopathy. Methods Thirty-seven thyroid-associated ophthalmopathy patients (73 eyes) had type A ultrasound results of 19 non-thyroid-related eye diseases (38 eyes). The results were compared with clinical manifestations and analyzed statistically. Results The sensitivity and specificity of type A ultrasound in the diagnosis of thyroid-associated ophthalmopathy were 91 78% and 89 4 7% respectively. The thickness of rectus muscle in patients with thyroid-associated ophthalmopathy was significantly increased, which was significantly different from non-thyroid-related ophthalmopathy All P <0 0 1). The data of this group mainly consisted of multiple rectus muscles, 57 53% of which were affected by 4 rectus muscles and 54.8% by 1 muscle. The frequency of upper rectus muscle was the highest (90 4 1%). The square sum of the thickness of each rectus muscle was highly correlated with the protrusion of the eye (r = 0 78, P 0 01). The ultrasound of the 9 patients (18 eyes) > 6 months) The results showed that the rectus muscle thickness showed a downward trend. Conclusion A-type ultrasound can provide accurate quantitative indicators for the early diagnosis of thyroid-related ophthalmopathy, and provide important evidence for the differential diagnosis and treatment options.
