目的:探讨2012~2013年平阳县166例妇女宫颈病变的发病情况,为宫颈病变的防治提供指导。方法:选择2012年10月~2013年10月该院门诊收治的166例妇女进行宫颈薄层液基细胞学检查(TCT)、HPVL1壳蛋白检查、阴道镜检查和宫颈活检,分析妇女宫颈病变的发病情况。结果:166例受检者中,经阴道镜检查与宫颈活检确诊宫颈上皮内瘤变(CIN)95例,其中CINⅠ级56例、CINⅡ级29例、CINⅢ级9例、宫颈鳞癌1例。TCT检查85例异常,其中不典型鳞状上皮细胞(ASCUS)36例、低度鳞状上皮内病变(LSIL)25例、高度鳞状上皮内病变(HSIL)23例、鳞状上皮细胞癌抗原(SCC)1例,TCT检查筛查宫颈病变灵敏度为82.11%。95例CIN患者中,HPVL1壳蛋白阳性86例,HPVL1壳蛋白检测筛查宫颈病变灵敏度为90.52%,特异度为88.73%。HPVL1壳蛋白检测的灵敏度明显高于TCT检查(P<0.05)。CINⅠ病变患者中HPVL1壳蛋白阳性率为98.21%,CINⅡ为89.66%,CINⅢ为55.55%。结论:该院门诊妇女宫颈病变的发病率较高,应加强妇女的生殖健康教育,预防宫颈病变。HPVL1壳蛋白检测技术在异常细胞的检出率高,操作简单方便,对妇女宫颈病变的筛查与防治有重要的应用价值。
Objective: To investigate the incidence of cervical lesions in 166 women from 2012 to 2013 in Pingyang County, and to provide guidance for the prevention and treatment of cervical lesions. Methods: A total of 166 women underwent outpatient treatment from October 2012 to October 2013 in our hospital for cervical cytology (TCT), HPV L1 capsid protein, colposcopy and cervical biopsy. The prevalence of cervical lesions Incidence. Results: Among the 166 subjects, 95 cases of cervical intraepithelial neoplasia (CIN) were diagnosed by colposcopy and cervical biopsy. Among them, 56 cases were CIN Ⅰ, 29 cases CIN Ⅱ, 9 cases CIN Ⅲ and 1 case squamous cell carcinoma of the cervix. Totally 85 cases were abnormal by TCT, including 36 cases of ASCUS, 25 cases of low grade squamous intraepithelial lesion (LSIL), 23 cases of high grade squamous intraepithelial lesion (HSIL), squamous cell carcinoma antigen (SCC) in 1 case. The sensitivity of TCT in screening cervical lesions was 82.11%. Of the 95 CIN patients, 86 were positive for HPVL1 capsid protein, 90.52% for HPVL1 capsid protein and 88.73% for cervical lesions. The sensitivity of the HPVL1 capsid protein test was significantly higher than that of the TCT test (P <0.05). The positive rate of HPVL1 capsid protein was 98.21% in CINⅠpatients, 89.66% in CINⅡ and 55.55% in CINⅢ. Conclusion: The incidence of cervical lesions in outpatient clinics is high. Women’s reproductive health education should be strengthened to prevent cervical lesions. Detection of HPVL1 capsid protein in abnormal cells with high detection rate, easy to operate, the screening of women with cervical lesions and prevention and treatment have important application value.
