本文报告了在北京市10个区县703.2万人口地区(含35所医院),使用标化乙肝血源疫苗和乙肝免疫球蛋白,对HBV不同感染情况孕妇的新生儿,采用不同免疫剂量、方案和不同免疫对策的研究结果。对HBsAg和HBeAg双阳性母亲的新生儿采用HBIG×2+20μg×3保护效果可达97.13%,为最佳免疫方案。对HBsAg单阳性母亲的新生儿仅接种较大剂量疫苗即可得到很好保护。对HBsAg阴性母亲的新生儿以按种20-10-10疫苗为宜。从而提出了北京地区应采取“在筛选基础上的全免策略”。
This article reports the use of standardized blood hepatitis B vaccine and hepatitis B immunoglobulin in 70.32 million population areas (including 35 hospitals) in 10 districts and counties in Beijing. Different doses of HBV are used in newborns of pregnant women with different HBV infection status. And the results of different immunization strategies. HBsAg and HBeAg-positive mothers of newborns with HBIG × 2 +20μg × 3 protection effect up to 97.13%, the best immunization program. Newborns with HBsAg single positive mothers are protected only with larger doses of vaccine. Neonates of HBsAg-negative mothers should be vaccinated according to 20-10-10. Therefore, it is proposed that Beijing area should adopt “free strategy based on screening”.
