目的研究中、重度牙周炎患者龈沟液中RANKL、OPG的浓度及经牙周基础治疗后龈沟液中RANKL、OPG及其比值的变化,以期为临床提供参考。方法随机选取40例中、重度牙周炎患者为牙周病组进行牙周基础治疗,采集治疗前及治疗后3周龈沟液,测量所含RANKL、OPG浓度,同期检查40例健康者作为对照组。结果 1治疗前牙周病组GCF中RANKL浓度为53.13±l4.43pg/nl,OPG的浓度为69.86±20.98 pg/nl,RANKL/OPG比值为1.57±1.18;对照组GCF中RANKL浓度为28.90±12.57pg/nl,OPG的浓度为109.08±49.35 pg/nl,RANKL/OPG比值为0.33±0.25。三组数值差异均具统计学意义(P<0.01)。2经牙周治疗后牙周病组患者RANKL浓度降为40.56±9.63;OPG浓度提高为88.96±17.24pg/nl;RANKL/OPG比值为0.44±0.32,较治疗前均具有统计学意义。3牙周病组患者治疗前后的AL和PD的变化与GCF中RANKL浓度变化呈正相关关系(相关系数分别为r=0.586,P=0.032;r=0.601,P=0.031);与OPG浓度变化无明显相关性(相关系数分别为r=-0.176,P=0.532;r=-0.187,P=0.519)。结论 1慢性牙周炎患者GCF中RANKL的含量升高,OPG的含量降低,提示牙槽骨的吸收可能与牙周局部RANKL和OPG的比例失调有关。2基础治疗对牙周病控制有一定作用。3可考虑干预OPG/RANKL/RANK通路影响牙槽骨的改建,从而找到更有针对性的牙周炎治疗方法。
Objective To study the changes of RANKL and OPG in gingival crevicular fluid and the changes of RANKL, OPG and their ratio in gingival crevicular fluid after periodontal treatment in patients with moderate and severe periodontitis, so as to provide reference for clinic. Methods Forty patients with moderate or severe periodontitis were randomly divided into periodontal group and periodontal group. The levels of RANKL and OPG in the gingival crevicular fluid before and after 3 weeks of treatment were collected and 40 healthy subjects were examined simultaneously Control group. Results 1 The concentration of RANKL in GCF of pretreatment period was 53.13 ± 14.43pg / nl, the concentration of OPG was 69.86 ± 20.98 pg / nl, the ratio of RANKL / OPG was 1.57 ± 1.18; the concentration of RANKL in control group was 28.90 ± 12.57 pg / nl, the OPG concentration was 109.08 ± 49.35 pg / nl, and the RANKL / OPG ratio was 0.33 ± 0.25. The three groups of differences were statistically significant (P <0.01). 2 After periodontal treatment, the concentration of RANKL decreased to 40.56 ± 9.63, the concentration of OPG increased to 88.96 ± 17.24pg / nl, the ratio of RANKL / OPG was 0.44 ± 0.32, which was significantly higher than that before treatment. CONCLUSIONS1 The content of RANKL and the content of OPG in GCF of patients with chronic periodontitis are decreased, suggesting that the alveolar bone absorption may be related to the imbalance of RANKL and OPG in periodontal. 2 basic treatment of periodontal disease control has a certain effect. 3 may consider the intervention of OPG / RANKL / RANK pathway affect alveolar bone remodeling, to find more targeted periodontitis treatment.
