目的探讨成批伤食物中毒急救人力资源管理模式的构建及用Extend Sim软件仿真实践优化的效果。方法对2009年7月—2014年6月本地区6家三级甲等医院成批伤患者1 884例,成批伤事件的主要类型之一食物中毒,进行急救人力资源管理模式的构建仿真实践,找出瓶颈因素,多次改进模式相应参数,找出最佳优化模式,同种方法仿真不同伤员例数的优化急救资源模式。推算出急救人力资源与伤员的最佳比例。结果食物中毒成批伤7例伤员同时送到医院急诊科,在保障伤员成功率的前提下,最佳急救人力资源模式比初次设定模式的资源利用率由22.49%提高到34.96%,伤员与急救人力资源的最佳比例为轻伤员∶护士∶医生为5∶1∶1;重伤员:护士:医生:辅医为3∶3∶1∶1;危重伤员∶护士∶医生∶辅医为1∶2∶1∶1。结论成批伤食物中毒急救资源管理模式的构建及仿真实践优化,推算出急救人力资源与伤员的最佳比例能为管理者的资源管理决策提供可靠依据。
Objective To explore the construction of emergency human resource management model for batch wounds and the effect of optimization using Extend Sim software. Methods From July 2009 to June 2014, there were 1 884 batch trauma patients, one of the major types of batch wounds, and 6 emergency medical resource management models , Identify the bottleneck factors, improve the model parameters several times to find the best optimization model, the same method to simulate the number of different injuries optimized emergency resources mode. Calculate the best ratio of first aid human resources to the wounded. Results The seven cases of casualties caused by food poisoning were sent to the hospital emergency department at the same time. Under the premise of guaranteeing the success rate of the wounded, the resource utilization ratio of the best first aid human resource model to that of the initial setting mode increased from 22.49% to 34.96% The best proportion of emergency human resources for the light casualties: nurses: doctors for the 5: 1: 1; seriously injured: nurses: doctors: supplementary medicine for the 3: 3: 1: 1; critically ill casualties: nurses: doctors: 1: 2: 1: 1. Conclusion The construction of emergency management model of food poisoning batches and the optimization of simulation practice, and deducing the best proportion of first aid human resources and wounded can provide a reliable basis for managers’ resource management decision-making.
