目的:对比研究超声弹性应变率比值法与评分法鉴别乳腺结节良恶性的应用价值。方法:选择2013年6月至2014年3月72例乳腺结节患者的78处病灶进行超声弹性成像检查,测量其应变率比值并进行弹性评分,构建受试者工作特征(ROC)曲线,以病理诊断结果作为金标准,对结果进行整理分析。结果:超声弹性应变率比值的曲线下面积为0.925,灵敏度、特异度是92.50%、92.11%,弹性评分法的曲线下面积是0.891,灵敏度、特异度是85.00%、89.47%。两者联合的灵敏度、特异度分别为95.00%、94.74%,高于单独任何一种方法。结论:超声弹性应变率比值是一个定量指标,能客观反映乳腺病灶的硬度情况,鉴别诊断价值高于弹性评分法,但两者联合诊断价值最高。
OBJECTIVE: To compare the value of ultrasonic elastic strain rate and scoring method in differentiating benign and malignant breast nodules. Methods: Seventy-two lesions of 72 patients with breast nodules from June 2013 to March 2014 were examined by echocardiography. The ratio of strain rate was measured and the elasticity score was calculated. The receiver operating characteristic (ROC) curve was constructed. Pathological diagnosis as the gold standard, the results were analyzed. Results: The area under the curve of the elastic strain rate ratio was 0.925. The sensitivity and specificity were 92.50% and 92.11%. The area under the curve of the elasticity score method was 0.891. The sensitivity and specificity were 85.00% and 89.47%. The sensitivity and specificity of the combination of the two were 95.00% and 94.74% respectively, higher than that of either method alone. Conclusion: The ratio of ultrasonic elastic strain rate is a quantitative index, which objectively reflects the hardness of breast lesions. The differential diagnosis value is higher than that of elastic score method, but the combined diagnosis value is the highest.
