目的:探讨p15、p16、Rb、p27、p53及PCNA基因蛋白在不同分期、不同部位和不同分级的喉鳞状细胞癌(LSCC)中的表达和临床意义。方法:用免疫组织化学染色SP法对65例LSCC进行染色,30例正常黏膜为对照组,检测p15、p16、Rb、p27、p53及PCNA基因蛋白的表达,并进行统计学分析。结果:6种基因蛋白的表达在正常黏膜和LSCC中的差异有统计学意义(均P<0.01)。且p15和p27表达不仅和LSCC生长部位相关,而且和临床分期、分化程度相关。p16蛋白表达与LSCC的病理分级呈负相关,PCNA蛋白的表达与LSCC的病理分级呈正相关,两者的表达均与临床分期和肿瘤生长部位无关。而LSCC生长越晚期,Rb、p53蛋白的表达,前者呈下降,后者呈上升状态,且p53蛋白的表达和恶性程度呈正相关。结论:p15和p27基因表达的缺失是LSCC发生发展的重要过程,对两者的检测在LSCC的治疗和预后方面有一定意义。p16和PCNA反映肿瘤细胞的增殖状态,与肿瘤的发生有关,与肿瘤的发展过程无关。而Rb、p53基因表达多与肿瘤的发展过程相关。
Objective: To investigate the expression and clinical significance of p15, p16, Rb, p27, p53 and PCNA in different stages, different parts and different grades of laryngeal squamous cell carcinoma (LSCC). Methods: 65 cases of LSCC were stained with immunohistochemical SP method and 30 cases of normal mucosa were taken as control group. The expressions of p15, p16, Rb, p27, p53 and PCNA were detected and statistically analyzed. Results: The expression of 6 kinds of genes in normal mucosa and LSCC were significantly different (all P <0.01). And p15 and p27 expression not only related to the growth of LSCC sites, but also with the clinical stage, the degree of differentiation related. The expression of p16 protein was negatively correlated with the pathological grade of LSCC. The expression of PCNA protein was positively correlated with the pathological grade of LSCC. The expression of p16 protein was not associated with the clinical stage and tumor growth site. The more advanced LSCC, Rb, p53 protein expression, the former decreased, the latter increased, and the expression of p53 protein and the degree of malignancy was positively correlated. CONCLUSION: The loss of p15 and p27 gene expression is an important process for the development of LSCC. The detection of both of them has a certain significance in the treatment and prognosis of LSCC. p16 and PCNA reflect the proliferation of tumor cells, and the occurrence of the tumor, has nothing to do with the development of the tumor.
