胃平滑肌肉瘤常可见到,但极少自发性破裂而危及生命。本文报告因胃外型平滑肌肉瘸自发性破裂引起腹腔内大出血而导致休克一例。患者,女,21岁。于1981年11月9日突然发生上腹剧裂疼痛4小时,向左肩部放射,伴噁心。两年前发生类似腹痛。体检:体温36.8℃,脉搏76次/分,血压100/80毫米汞柱。急性痛苦病容,巩膜无黄染。上腹及左上腹明显压痛,腹肌紧张。化检:白细胞12000,中性86％,尿淀粉酶128温氏单位。臆断:急性胰腺炎。入院后2小时腹痛更加
Gastric leiomyosarcoma is often seen, but rarely spontaneously ruptured and life-threatening. This article reports one case of shock due to intra-abdominal haemorrhage caused by spontaneous rupture of extra-abdominal smooth muscle spasm. Patient, female, 21 years old. On November 9, 1981, epigastric fissure pain suddenly occurred for 4 hours, radiating to the left shoulder with nausea. Similar abdominal pain occurred two years ago. Physical examination: body temperature 36.8°C, pulse 76 beats/minute, blood pressure 100/80 mm Hg. With acute pain, the sclera is yellowish. The upper abdomen and left upper abdomen were markedly tender and the abdominal muscles were tense. Chemical examination: WBC 12000, neutral 86%, urine amylase 128 Wen’s unit. Cut off: acute pancreatitis. 2 hours after admission is more abdominal pain
