目的观察颅内感染性疾病脑脊液中肝细胞生长因子(HGF)和可溶性细胞间粘附分子-1(sI- CAM-1)的水平变化并探讨其临床意义。方法采用双抗体酶联免疲吸附法(ELISA)分别测定病毒性脑炎37例、细菌性脑膜炎31例患者在急性期和恢复期脑脊液中HGF、sICAM-1的含量,并将抑郁症和紧张性头痛患者20例设为对照组进行比较。结果在急性期细菌性脑膜炎组脑脊液中HGF、sICAM-1的含量分别为(0.74±0.40)和(16.85±7.32)μg/L,与对照组[(0.23±0.16)和(2.31±2.08)μg/L]比较差异均有统计学意义(均P<0.01)。病毒性脑炎组脑脊液中HGF的含量为(0.37±0.24)μg/L,与对照组比较差异无统计学意义(P>0.05);sICAM-1的含量为(7.19±5.85)μg/L,与对照组比较差异有统计学意义(P<0.01)。脑脊液中HGF、sICAM-1的含量细菌性脑膜炎组与病毒性脑炎组比较差异均有统计学意义(均p<0.01)。在恢复期细菌性脑膜炎和病毒性脑炎脑脊液中HGF、sICAM-1的含量与对照组比较差异均无统计学意义(均P>0.05)。结论HGF、sICAM-1参与了颅内感染性疾病的病理过程。脑脊液中HGF、siCAM-1水平的测定有助于细菌性脑膜炎与病毒性脑炎的诊断及鉴别诊断。
Objective To observe the changes of hepatocyte growth factor (HGF) and soluble intercellular adhesion molecule-1 (sI-CAM-1) in cerebrospinal fluid (CSF) in patients with intracranial infectious diseases and to explore its clinical significance. Methods The levels of HGF and sICAM-1 in cerebrospinal fluid of 37 patients with viral encephalitis and 31 patients with bacterial meningitis during acute and convalescent cycles were determined by ELISA and ELISA respectively. The levels of depression and 20 cases of tension headache patients as control group for comparison. Results The levels of HGF and sICAM-1 in cerebrospinal fluid of patients with acute meningitis were (0.74 ± 0.40) and (16.85 ± 7.32) μg / L, respectively, compared with those in control group [(0.23 ± 0.16) and (2.31 ± 2.08) μg / L] were statistically significant differences (all P <0.01). The level of HGF in cerebrospinal fluid of patients with viral encephalitis was (0.37 ± 0.24) μg / L, which was not significantly different from that of the control group (P> 0.05). The content of sICAM-1 was (7.19 ± 5.85) Compared with the control group, the difference was statistically significant (P <0.01). The contents of HGF and sICAM-1 in cerebrospinal fluid were significantly different between bacterial meningitis group and viral encephalitis group (all p <0.01). The levels of HGF and sICAM-1 in cerebrospinal fluid of convalescent bacterial meningitis and viral encephalitis were not significantly different from those in control group (all P> 0.05). Conclusion HGF and sICAM-1 are involved in the pathological process of intracranial infectious diseases. Determination of cerebrospinal fluid HGF, siCAM-1 levels are helpful for the diagnosis and differential diagnosis of bacterial meningitis and viral encephalitis.
