目的观察脑缺血后再灌注损伤的兔在骨髓间充质干细胞移植术后脑磁共振灌注加权成像(MRPWI)的表现,探讨MRPWI在骨髓间充质干细胞治疗缺血性脑卒中实验研究中的应用价值。方法将雄性成年新西兰兔用随机数字表法分为假手术组(Sham组,n=20)、脑缺血再灌注组(MCAO组,n=30)和兔骨髓间充质干细胞治疗组(MSCs组,n=30),每组分别于造模成功后1、3、5、9d行PWI、DWI和常规MRI检查。每组均在造模成功后1、9d行神经功能损害程度评分,对每个检查时间点的缺血脑半球2个感兴趣区域(ROI)的脑组织血流量相对参量值(rCBF)进行统计,2个ROI基于DWI及PWI图像界定:两者均异常为1区(ROI-1)、DWI正常和PWI异常为2区(ROI-2)。结果 Sham组各时间点rCBF未见明显异常。与假手术组相比,MSCs治疗组与MCAO组的1区和2区rCBF在每个时间点均下降(P<0.05)。与MCAO组比较,每个时间点MSCs治疗组1区的rCBF差异均无统计学意义(P>0.05);MSCs治疗组1、3、5d的2区rCBF差异无统计学意义(P>0.05),9d的2区rCBF升高且具有统计学意义(P<0.05)。与1d相比,MSCs组与MCAO组9d的神经功能损害评分均减小,MSCs组减低最明显,与MCAO组比较差异有统计学意义(P<0.05)。结论 MRPWI能够反映骨髓间充质干细胞移植治疗脑缺血兔的脑组织微循环血流动力学的变化。
Objective To observe the MR perfusion weighted imaging (MRPWI) in rabbits with cerebral ischemia-reperfusion injury after bone marrow mesenchymal stem cell transplantation and to explore the role of MRPWI in the experimental study of bone marrow mesenchymal stem cells in the treatment of ischemic stroke Value. Methods Male New Zealand white rabbits were divided into sham operation group (n = 20), cerebral ischemia reperfusion group (n = 30) and MSCs group (MSCs) Group, n = 30). PWI, DWI and conventional MRI were performed on the 1st, 3rd, 5th and 9th day after successful modeling in each group. Each group was scored for neurological impairment at 1 and 9 days after successful modeling, and the relative parameters (rCBF) of cerebral blood flow in two regions of interest (ROI) of the ischemic hemisphere at each examination time were counted The two ROIs were defined on the basis of DWI and PWI images: both were abnormally zone 1 (ROI-1), normal DWI and PWI abnormal zone 2 (ROI-2). Results There was no obvious abnormality of rCBF in Sham group at all time points. Compared with the sham-operated group, rCBF in zone 1 and zone 2 of MSCs-treated group and MCAO group decreased at each time point (P <0.05). Compared with MCAO group, there was no significant difference in rCBF between group 1 and group 1 at each time point (P> 0.05). There was no significant difference in rCBF between group 1 and group 5 at day 1, , 2 rCBF of 9d increased and had statistical significance (P <0.05). Compared with the 1d group, the scores of neurological impairment in MSCs group and MCAO group decreased at 9th day, while those in MSCs group decreased the most obviously. There was significant difference between MCCs group and MCAO group (P <0.05). Conclusion MRPWI can reflect the change of hemodynamics in brain tissue after cerebral ischemia in bone marrow mesenchymal stem cells transplantation.
