目的探讨老年单纯收缩期高血压(ISA)患者脉压与同型半胱氨酸(HCY)水平和颈动脉内膜厚度(IMT)的相关性。方法选择2011年3月—2013年3月启东市中医院收治的45例老年ISA患者作为观察组,选取同期体检健康者45例作为对照组。观察两组患者脉压、HCY及血脂水平〔总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)〕和IMT。结果观察组脉压、HCY水平、IMT高于对照组,差异有统计学意义(P<0.05);两组患者TC、TG、LDL-C、HDL-C水平比较,差异有统计学意义(P<0.05)。老年ISA患者的脉压与HCY水平、IMT呈正相关(r=0.165、0.142,P<0.05)。结论老年ISA患者的脉压与HCY水平、IMT呈正相关,老年ISA患者血脂水平升高、脉压增大、HCY水平升高和IMT增加相关;脉压和HCY水平升高是老年ISA患者动脉粥样硬化的重要危险因子。
Objective To investigate the relationship between pulse pressure, homocysteine (HCY) and carotid intima-media thickness (IMT) in elderly patients with isolated systolic hypertension (ISA). Methods From March 2011 to March 2013, 45 elderly ISA patients admitted to Qidong Hospital of Chinese Medicine were selected as the observation group, and 45 healthy subjects were selected as the control group during the same period. Pulse pressure, HCY and blood lipid levels (TC, TG, LDL-C, HDL-C) and IMT were observed in two groups. Results The pulse pressure, HCY level and IMT in observation group were significantly higher than those in control group (P <0.05). The levels of TC, TG, LDL-C and HDL-C in two groups were significantly different (P <0.05). The pulse pressure of elderly patients with ISA was positively correlated with HCY level and IMT (r = 0.165,0.142, P <0.05). Conclusion The pulse pressure of elderly patients with ISA is positively correlated with the level of HCY and the level of IMT. The level of serum lipids, the increase of pulse pressure, the increase of HCY and the increase of IMT in elderly patients with ISA are higher. An important risk factor for sclerosis.
