目的探讨儿童配戴RGP镜片的有效性和安全性。方法3年来观察配戴RGP镜片48例(94眼)7-12岁屈光不正儿童。分析配戴前检查、验光、基弧设计,正确配戴健康教育及护理。观察配适、视力矫正效果和并发症。结果94眼屈光类型以复性近视散光居多,占59.5%。屈光度≥±10.00D的极重度屈光不正占25.5%。94眼框架眼镜矫正视力与首次配戴RGP镜片矫正视力对比,RGP镜片矫正视力明显优于框架眼镜(P<0.01)。94眼戴框架眼镜被诊断为弱视者有44眼。首次配戴RGP镜片矫正视力≥0.8者32眼(76.2%),其中有19眼(45.2%)配戴RGP镜片矫正视力在1.0以上,否定了弱视诊断。一次性配戴成功占89.6%。在3个月内丢失镜片占12.8%。无角膜上皮擦伤或角膜溃疡等并发症。结论RGP镜片矫正视力良好,特别是高度屈光不正,高度散光,高度屈光参差能获得较好矫正视力,对屈光不正性弱视有更好的矫正。儿童适合配戴RGP镜片,安全有效。安全性关键在于镜片良好的配适及配戴教育。镜片基弧的设计,配适的评估与成人有差异。要重视复诊时的体征,适当调整基弧至关重要。
Objective To explore the effectiveness and safety of children wearing RGP lenses. Methods In the past three years, 48 cases (94 eyes) with RGP lenses were observed, and 7-12 years old children with ametropia were examined. Analysis of pre-wear inspection, optometry, base arc design, the correct wear health education and care. Observe the fit, vision correction and complications. Results Ninety-four eyes of refractive type were mostly myopic astigmatism, accounting for 59.5%. Refractive index ≥ ± 10.00D 25.5% of very serious refractive errors. The corrective visual acuity of RGP lenses was superior to that of frame glasses (P <0.01) in 94 eyes with corrective vision and RGP lenses. There were 44 eyes of 94 eyes who were diagnosed with amblyopia wearing glasses. For the first time, RGP lenses were used to correct 32 eyes (76.2%) with visual acuity≥0.8. Among them, 19 eyes (45.2%) had RGP corrected visual acuity of 1.0 or above, which negated the diagnosis of amblyopia. One-time wear success 89.6%. Lost lenses in 3 months accounted for 12.8%. No corneal abrasion or corneal ulcer and other complications. Conclusions RGP lenses have good visual acuity, especially high refractive error, high astigmatism and high anisometropia, and better corrected visual acuity and better correction of refractive amblyopia. Suitable for children wearing RGP lenses, safe and effective. The key is the safety of the lens fit and wear education. Lens base arc design, fitness appraisal and adults are different. To pay attention to the signs of referral, the appropriate adjustment of the base arc is essential.
