目的:观察前列地尔联合丁苯酞治疗老年2型糖尿病合并认知功能障碍的治疗效果,为2型糖尿病合并认知功能障碍老年患者的临床治疗提供帮助。方法:选取临床收治的2型糖尿病合并认知功能障碍老年患者100例,按随机数字表法分为:观察组(50例)和对照组(50例),对照组老年患者给予常规治疗,在此基础上,观察组老年患者给予丁苯酞和前列地尔的联合治疗,检测治疗前后2组老年患者MoCA评分、血小板参数、血液动力学及神经因子的变化。结果:与治疗前相比,2组老年患者神经因子(S-100β和NSE)、血小板参数(PDW、MPV、PLCR和PCT)和R1显著降低,MoCA评分、神经因子(NGF和NTF)和血液动力学指标(v_(max)、v_(min)和Q)显著升高,差异存在统计学意义(P<0.05)。经过联合治疗后观察组老年患者MoCA评分、神经因子(NGF和NTF)和血液动力学指标(v_(max)、v_(min)和Q)显著高于治疗后对照组,神经因子(S-100β和NSE)、血小板参数(PDW、MPV、PLCR和PCT)和R1显著低于治疗后对照组,差异存在统计学意义(P<0.05)。结论:丁苯酞和前列地尔的联合治疗能够改善2型糖尿病合并认知功能障碍老年患者的血小板状况,提高患者的MoCA评分和神经机能同时加速血液流动,对2型糖尿病合并认知功能障碍老年患者的临床治疗具有重要的意义。
Objective: To observe the therapeutic effect of alprostadil combined with butylphthalide in the treatment of senile type 2 diabetes mellitus complicated with cognitive dysfunction, and to help the clinical treatment of elderly patients with type 2 diabetes mellitus complicated with cognitive impairment. Methods: A total of 100 elderly patients with type 2 diabetes mellitus complicated with cognitive impairment were enrolled in this study. They were divided into observation group (50 cases) and control group (50 cases) by random number table. Patients in control group were given routine treatment. On this basis, the elderly patients in the observation group were given combination therapy of butylphthalide and alprostadil, and the changes of MoCA score, platelet parameters, hemodynamics and neurokines in the two groups were detected before and after treatment. Results: Compared with those before treatment, the neurological scores (S-100β and NSE), platelet parameters (PDW, MPV, PLCR and PCT) and R1 were significantly decreased in the elderly patients. MoCA score, NGF and NTF, The kinetic parameters (v_ (max), v_ (min) and Q) were significantly increased, the difference was statistically significant (P <0.05). MoCA score, neurological factors (NGF and NTF) and hemodynamic indexes (v max, v min and Q) in the observation group were significantly higher than those in the control group, neurological factors (S-100β And NSE), platelet parameters (PDW, MPV, PLCR and PCT) and R1 were significantly lower than those of the control group after treatment, the difference was statistically significant (P <0.05). CONCLUSIONS: The combination of butylphthalide and alprostadil improves platelet status in elderly patients with type 2 diabetes mellitus complicated with cognitive impairment, improves patient’s MoCA score and neurologic function while accelerating blood flow, and increases cognitive impairment in type 2 diabetes mellitus Clinical treatment of elderly patients is of great significance.
