目的研究肝炎肝硬化患者心电图QTc间期、Tp-e间期和Tp-e/QT的改变,从而探讨肝炎肝硬化患者QTc间期、Tp-e间期和Tp-e/QT的临床意义。方法选取2011年12月至2012年3月住院的肝炎肝硬化患者156例,对其QTc间期、Tp-e间期和Tp-e/QT进行分析;另设健康体检者66例为对照组。结果肝炎肝硬化患者与对照组心电图异常率、QTc间期、Tp-e间期比较,差异有统计学意义(P<0.01);Tp-e/QT比较差异无统计学意义(P>0.05);肝炎肝硬化ChildA、B、C级三组间比较,心电图异常率差异无统计学意义(P>0.05);QTc间期、Tp-e间期与Child分级呈正相关(P<0.01)。肝炎肝硬化电解质正常组与异常组比较,QTc间期差异非常显著(P<0.01);心电图异常率、Tp-e间期、Tp-e/QT差异无统计学意义(P>0.05)。结论肝炎肝硬化患者心电图异常率明显升高,电解质紊乱使QTc间期延长显著。QTc间期、Tp-e间期可以作为肝炎肝硬化患者病情严重程度判断的辅助参考指标,临床医师应密切注意并及时治疗。
Objective To investigate the changes of QTc interval, Tp-e interval and Tp-e / QT in patients with liver cirrhosis and to explore the clinical significance of QTc interval, Tp-e interval and Tp-e / QT in patients with liver cirrhosis. Methods 156 patients with cirrhosis of liver cirrhosis hospitalized from December 2011 to March 2012 were selected to analyze the QTc interval, Tp-e interval and Tp-e / QT. Another 66 healthy subjects were selected as the control group . Results There were significant differences in ECG abnormality rate, QTc interval and Tp-e interval between patients with hepatitis cirrhosis and control group (P <0.01). There was no significant difference in Tp-e / QT between the two groups (P> 0.05) There was no significant difference in ECG abnormalities between ChildA, B and C groups of hepatitis cirrhosis (P> 0.05). There was a positive correlation between QTc interval and Tp-e interval and Child classification (P <0.01). There were significant differences in QTc interval between normal liver cirrhosis electrolyte group and abnormal group (P <0.01). Electrocardiogram abnormality rate, Tp-e interval and Tp-e / QT had no statistical significance (P> 0.05). Conclusion The abnormal rate of electrocardiogram in patients with cirrhosis of liver is obviously increased. The disturbance of electrolytes makes the interval of QTc prolong significantly. QTc interval, Tp-e interval can be used as a secondary reference index for judging the severity of hepatitis cirrhosis patients, clinicians should pay close attention to and timely treatment.
