目的对比研究Ⅰ类错畸形患者应用拔牙矫治与非拔牙矫治对口咽气道容积的影响。方法选取2010年5月至2013年3月接受错畸形矫治的97位Ⅰ类错畸形患者作为研究对象。根据矫治方法不同,分为实验组(拔牙矫治)和对照组(非拔牙矫治)。利用回顾性分析方法,收集患者矫治后的基础临床资料以及影像学资料。结果组内比较:两组患者矫治后面中部长度、下颌骨长度较矫治前长;拔牙矫治组矫治后上中切牙倾角、下中切牙倾角较矫治前减小。非拔牙矫治组矫治后上中切牙倾角、下中切牙倾角较矫治前增大。矫治前后各项指标差异均具有统计学意义(P<0.05)。组间比较:矫治前两组各项影像学指标差异无统计学意义(P>0.05),但矫治后拔牙矫治组面中部长度、下颌骨长度更短于非拔牙矫治组;上中切牙倾角、下中切牙倾角小于非拔牙矫治组;口咽气道容积及最狭窄部位面积均大于非拔牙矫治组。矫治后两组各项指标差异均具有统计学意义(P<0.05)。结论拔牙矫治在Ⅰ类错畸形的治疗中,对口咽气道容积的影响更为积极,应在Ⅰ类错畸形的治疗中引起足够重视。
Objective To compare the effects of extraction and non-extraction on the oropharyngeal airway volume in patients with type Ⅰ malformations. Methods A total of 97 patients with class Ⅰ malocclusion undergoing correction of malocclusion were selected from May 2010 to March 2013 as the research object. According to different treatment methods, divided into experimental group (extraction treatment) and control group (non-extraction treatment). The retrospective analysis was used to collect the basic clinical data and imaging data of patients after correction. Results In-group comparison: The length of the middle jaw and the length of the mandible were longer in the two groups than that before the treatment. The inclination of the upper incisors and the incisors of the lower incisors were lower than those before the orthodontic treatment. Non-extractive group orthodontic treatment of upper incisors incisors, lower incisors incline more than before correction. Before and after correction of various indicators were statistically significant (P <0.05). There was no significant difference between the two groups in imaging parameters (P> 0.05), but the length of mandibular surface in mandibular extraction group was shorter than that in non-extraction group after orthodontic treatment. The incisive angle , The incisors of lower incisors were less than non-extracorporeal patients, and the areas of oropharyngeal airways and the narrowest were larger than that of non-extracussion group. After treatment, the differences between the two groups were statistically significant (P <0.05). Conclusion In the treatment of class Ⅰ malformation deformity, the effect of extraction treatment on oropharyngeal airway volume is more positive. It should pay enough attention in the treatment of class Ⅰ malformation.
