目的:探讨声门上喉癌、下咽癌瘤体、周围组织及转移淋巴结真核细胞翻译起始因子4E(eIF4E)、基质金属蛋白酶-9(MMP-9)的表达情况,以及2种分子标志物表达的相关性。方法:采用免疫组织化学SP法及图像分析,检测38例肿瘤瘤体及不同距离手术切缘组织、转移淋巴结中eIF4E、MMP-9的表达情况。结果:①在喉及下咽部鳞状细胞癌组织中MMP-9阳性表达率为68.4%,表达强度与淋巴结转移、临床分期相关,而与肿瘤位置及病理分级无关。MMP-9在声门上喉癌、下咽癌瘤体及5mm、10mm切缘组织中的表达阳性率依次递减;②喉及下咽部鳞癌组织eIF4E表达阳性率为89%,表达强度与淋巴结转移、肿瘤病理分级、临床分期相关。eIF4E在声门上喉癌、下咽癌瘤体及5mm、10mm切缘组织表达阳性率依次递减;③MMP-9和eIF4E在正常淋巴结中均无表达,在转移淋巴结表达阳性率分别为28%和42%;④eIF4E与MMP-9表达无相关性。结论:eIF4E、MMP-9在喉癌及下咽癌瘤体中的表达高于癌周正常组织,eIF4E在瘤体及癌周正常组织表达阳性率高于MMP-9。eIF4E可作为预测喉癌及下咽癌浸润生长、转移的敏感标志物。
Objective: To investigate the expression of translation initiation factor 4E (eIF4E) and matrix metalloproteinase-9 (MMP-9) in supraglottic carcinoma, hypopharyngeal carcinoma, peripheral tissues and lymph node metastasis, Relevance of marker expression. Methods: Immunohistochemical SP method and image analysis were used to detect the expression of eIF4E and MMP-9 in 38 cases of tumor and their adjacent tissues at different distances. Results: ① The positive expression rate of MMP-9 in laryngeal and hypopharyngeal squamous cell carcinoma tissues was 68.4%. The expression intensity was correlated with lymph node metastasis and clinical stage, but not with tumor location and pathological grade. The positive expression rates of MMP-9 in supraglottic carcinoma, hypopharyngeal carcinoma, and 5 mm, 10 mm marginal tissues decreased successively. ② The positive expression rate of eIF4E in laryngeal and hypopharyngeal squamous cell carcinoma tissues was 89% Lymph node metastasis, tumor pathological grade, clinical stage related. eIF4E in the supraglottic laryngeal cancer, hypopharyngeal carcinoma and 5mm, 10mm margin of the positive rate of tissue expression in descending order; ③MMP-9 and eIF4E in normal lymph nodes were not expressed in the positive rate of metastatic lymph nodes were 28% and 42%; ④eIF4E and MMP-9 expression was not related. Conclusion: The expression of eIF4E and MMP-9 in laryngeal carcinoma and hypopharyngeal carcinoma is higher than that in normal tissues. The positive expression rate of eIF4E in tumor and normal tissue is higher than that in MMP-9. eIF4E can be used as a sensitive marker for predicting invasion and metastasis of laryngeal carcinoma and hypopharyngeal carcinoma.
