目的探讨高效抗逆转录病毒联合免疫调节对艾滋病患者炎性因子及免疫功能的影响。方法选择2011年8月至2014年12月湖北省黄冈市疾病预防控制中心收治的艾滋病患者38例为研究对象,根据入院时间先后顺序随机分为观察组(n=19)和对照组(n=19)。对照组患者接受HAART治疗(齐多夫定300 mg、拉米夫定150 mg、奈韦拉平200 mg口服,2次/d),观察组患者在接受HAART治疗的基础上联合胸腺五肽治疗,两组均连续治疗24周。比较两组患者炎性因子、免疫功能、生活质量。结果治疗前两组患者炎性因子、免疫功能及生活质量等方面比较差异无统计学意义;治疗后均有不同程度的改善,与治疗前比较差异均有统计学意义,且观察组患者血清白细胞介素-17(IL-17)、白细胞介素-23(IL-23)含量明显高于对照组,C反应蛋白(CRP)含量明显低于对照组(P<0.05);CD4+T细胞、Th17细胞含量明显高于对照组,CD8+T细胞含量、Treg细胞明显低于对照组(P<0.05);生活质量:观察组心理、独立等评分明显高于对照组(t=2.078~3.359,P<0.05)。结论高效抗逆转录病毒联合免疫调节能够增强免疫功能,减轻炎症反应,改善生活质量。
Objective To investigate the effect of combined anti-retroviral immunotherapy on inflammatory cytokines and immune function in AIDS patients. Methods Thirty-eight AIDS patients admitted to Huanggang Municipal Center for Disease Control and Prevention in Hubei Province from August 2011 to December 2014 were randomly divided into observation group (n = 19) and control group (n = 19). Patients in the control group were treated with HAART (zidovudine 300 mg, lamivudine 150 mg, nevirapine 200 mg orally twice daily), and patients in the observation group were treated with thymopentin on the basis of HAART. Two groups Continuous treatment for 24 weeks. Inflammatory factors, immune function and quality of life were compared between the two groups. Results There was no significant difference in the inflammatory factors, immune function and quality of life between the two groups before treatment. After treatment, the levels of inflammatory cytokines, immune function and quality of life were all improved to some extent, which were significantly different from those before treatment, and the serum leukocyte The contents of IL-17 and IL-23 were significantly higher than those of the control group, and the contents of C-reactive protein (CRP) were significantly lower than those of the control group (P <0.05) The levels of Th17 cells in the observation group were significantly higher than those in the control group (P <0.05). The levels of CD8 + T cells and Treg cells in the observation group were significantly higher than those in the control group (t = 2.078-3.359, P <0.05). Conclusion Highly effective antiretroviral combined immunomodulation can enhance immune function, reduce inflammation and improve quality of life.
