目的探索福建汉族人群TCF7L2基因多态性以及基因-环境交互作用与2型糖尿病(T2DM)的关联性。方法采用1∶1匹配病例对照研究方法,选取福建省5个国家疾病监测点社区人群中2型糖尿病患者298例和对照者298例,进行问卷调查、体格测量和实验室检测。采用MALDI-TOF-MS技术检测TCF7L2基因的10个SNPs位点(rs11196205、rs12255372、rs4506565、rs7901695、rs7903146、rs7895340、rs11196218、rs12775879、rs290481、rs6585205)的基因型,采用SHEsis在线软件分析基因型分布频率和单体型,应用广义多因子降维法(GMDR)分析基因-环境交互作用,非条件logistic回归模型进行基因-环境交互作用危险性分析,Andersson Excel表计算相加交互作用指标:相对超额危险度比(RERI)、归因比(AP)和交互作用指数(S)。结果 TCF7L2基因10个SNP位点在病例组与对照组之间差异无统计学意义;向心性肥胖为T2DM的主效应影响因素;HOMA-β在病例组rs290481位点的三种基因型中的差异有统计学意义(P<0.05)。结论向心性肥胖对T2DM起主要影响作用;TCF7L2基因rs290481多态性可能影响胰岛β细胞胰岛素分泌功能。
Objective To explore the association between TCF7L2 gene polymorphism and gene-environment interaction and type 2 diabetes mellitus (T2DM) in Han nationality in Fujian province. Methods A 1: 1 matched case-control study was conducted. A total of 298 type 2 diabetic patients and 298 controls were enrolled in the questionnaire survey, physical measurement and laboratory tests in 5 community disease surveillance sites in Fujian Province. Genotypes of ten SNPs (rs11196205, rs12255372, rs4506565, rs7901695, rs7903146, rs7895340, rs11196218, rs12775879, rs290481, rs6585205) of the TCF7L2 gene were detected by MALDI-TOF-MS. The frequency of genotype distribution was analyzed by SHEsis online software And haplotype, gene-environment interaction using generalized multi-factor dimensionality reduction (GMDR), risk analysis of gene-environment interaction using non-conditional logistic regression model, Andersson Excel table calculation of additive interaction indicators: relative excess risk RERI, Attribution Ratio (AP) and Interaction Index (S). Results There were no significant differences in the 10 SNP loci of TCF7L2 gene between the case group and the control group. The central obesity was the main factor influencing T2DM. The difference of HOMA-β among the three genotypes of rs290481 in the case group There was statistical significance (P <0.05). Conclusion Concentric obesity has a major effect on T2DM. The rs290481 polymorphism of TCF7L2 gene may affect the insulin secretion of pancreatic β cells.
