乳果糖和新霉素是治疗门体性脑病的有效药物,这两种药物的临床疗效与动脉血氨的下降有关。但不能肯定这两种药物合用时,对降低血氨的浓度是否有相加作用。为此作者通过测定尿素合成率、降解率及肠内尿素清除率来估价两药合用时对肠道产氨的影响。研究对象为6例经活检证实的酒精性肝硬化患者,6例年龄52~65岁,均无腹水、水肿,脑病或胃肠道出血。在观察期间,除口服新霉素和乳果糖外,不用其它药物。试验前至少6个月内无饮酒史,常规肝功能试验正常或仅轻度异常。所有病人都在监护代谢情况下进行,严格控制饮食的质和量。
Lactulose and neomycin are effective drugs for the treatment of portal encephalopathy, the clinical efficacy of these two drugs and arterial ammonia decreased. But can not be sure whether these two drugs combined to reduce the concentration of ammonia or have an additive effect. To this end, the author estimates the rate of intestinal ammonia production by measuring the rate of urea synthesis, degradation rate and intestinal clearance of urea. The subjects were 6 patients with alcoholic liver cirrhosis confirmed by biopsy and 6 patients aged 52-65 years. No ascites, edema, encephalopathy or gastrointestinal bleeding were found. During the observation, in addition to oral neomycin and lactulose, no other drugs. At least 6 months before the test no history of alcohol consumption, routine liver function tests were normal or only mild abnormalities. All patients are under custody of metabolic conditions, strict control of diet quality and quantity.
