[目的]探讨超声弹性成像应变率比值在鉴别甲状腺良恶性结节中的应用价值。[方法]对70例甲状腺结节患者检出的87个结节进行超声弹性成像检查,并测量弹性成像应变率比值,绘制ROC曲线图,确定区分甲状腺良恶性结节应变率比值的临界点。[结果]甲状腺恶性结节SR比值平均为5.30±1.26,良性结节SR比值平均为2.79±1.19,两组间差异具有统计学意义(P<0.05)。甲状腺结节弹性应变率比值的ROC曲线下面积为0.936,选择约登指数最大切点0.71为临界点,对应的SR比值为3.63,其对应的灵敏度为97.3%,特异性为73.5%。[结论]超声弹性成像应变率比值能客观地反映甲状腺结节的相对硬度,有助于甲状腺良恶性结节的鉴别诊断,在临床诊断中具有较高的应用价值。
[Objective] To investigate the value of ultrasound elastography strain rate in the differential diagnosis of benign and malignant thyroid nodules. [Methods] Totally 87 nodules from 70 patients with thyroid nodules were examined by echography, and the ratio of elastic imaging strain rate was measured. The ROC curve was drawn to determine the critical point to distinguish the ratio of benign and malignant thyroid nodules. [Results] The average SR ratio of malignant thyroid nodules was 5.30 ± 1.26 and the average ratio of benign nodules was 2.79 ± 1.19. The difference between the two groups was statistically significant (P <0.05). The area under the ROC curve of the elastic strain rate ratio of thyroid nodules was 0.936. The maximum cut point of Yo’den index of 0.71 was selected as the critical point. The corresponding SR ratio was 3.63. The corresponding sensitivity was 97.3% and the specificity was 73.5%. [Conclusion] The ratio of ultrasound elastography strain rate can objectively reflect the relative hardness of thyroid nodules and contribute to the differential diagnosis of benign and malignant thyroid nodules. It has high application value in clinical diagnosis.
