目的探讨鼻咽癌放疗后鼻窦炎的鼻内镜手术治疗方法。方法对43例鼻咽癌放疗后并发鼻窦炎患者,经鼻腔冲洗及药物保守治疗1年以上未愈,采用鼻内镜下手术治疗,术后随访1年,观察其疗效。结果43例中,治愈22例(51.1%),好转20例(46.5%),无效1例(2.3%),总有效率97.7%。19例(44.2%)发生鼻腔粘连,经定期鼻内镜检查鼻腔清理而治愈。结论鼻内镜下手术治疗放疗后鼻窦炎疗效显著,术后定期随访并作鼻腔清理可防止鼻腔粘连及复发。
Objective To investigate the method of nasal endoscopic surgery for nasosinusitis after radiotherapy of nasopharyngeal carcinoma. Methods 43 patients with nasopharyngeal carcinoma after radiotherapy complicated with sinusitis were treated with nasal lavage and conservative treatment for more than 1 year. Surgical treatment was performed by endoscopic sinus surgery. The follow-up was 1 year after operation to observe the curative effect. Results Among 43 cases, 22 cases were cured (51.1%), 20 cases (46.5%) were improved, 1 case (2.3%) was ineffective and the total effective rate was 97.7%. 19 cases (44.2%) nasal adhesions, nasal clearance by regular endoscopic examination and cure. Conclusions Endoscopic sinus surgery has significant curative effect on sinusitis after radiotherapy. Regular postoperative follow-up and nasal clearance can prevent nasal adhesions and recurrence.
