目的探讨磁共振全身扩散加权成像(WB-DWI)技术对恶性肿瘤骨转移的诊断价值。方法采用WB-DWI方法检查32例门诊健康体检者和30例恶性肿瘤(乳腺癌、肺癌、肝癌、结肠癌、前列腺癌和卵巢癌)骨转移患者,利用3D最大密度投影(3D-MIP)重建及黑白翻转技术观察肿瘤骨转移病变显示效果。观察WB-DWI检测病灶部位及阳性病例数,并与单光子发射计算机断层成像术(SPECT)影像表现予以比较和相应分析。结果 WB-DWI检测阳性病例为30例,受累区域89处,共检出病灶数为185处;SPECT阳性病例为28例,受累区域75处,共检出病灶数为167处。结论WB-DWI检查技术具有诊断覆盖面积广泛、耗时短、价格低廉、无创伤性和检出率较高等优点,在恶性肿瘤骨转移检出和治疗后随访等方面具有重要的临床价值。
Objective To investigate the diagnostic value of magnetic resonance diffusion-weighted whole-body imaging (WB-DWI) in the diagnosis of malignant bone metastases. Methods 32 cases of out-patient health examination and 30 cases of malignant tumor (breast, lung, liver, colon, prostate and ovarian cancer) bone metastases were examined by WB-DWI method and reconstructed by 3D maximum density projection (3D-MIP) And black and white flip technology to observe the tumor bone metastases lesions show effect. The number of lesions detected by WB-DWI and the number of positive cases were observed and compared with single photon emission computed tomography (SPECT) imaging. Results There were 30 positive cases detected by WB-DWI in 89 affected areas, of which 185 were detected. The number of SPECT positive cases was 28 and the affected area was 75, with a total of 167 lesions detected. Conclusion WB-DWI has the advantages of wide diagnostic coverage, short time-consuming, low cost, non-invasiveness and high detection rate. It has important clinical value in detection of bone metastases and follow-up after treatment.
