目的对中药治疗慢性阻塞性肺疾病(chronic obstructive pulmonary diseases,COPD)急性加重期的效果进行评价。方法选择2011年1月—2014年1月就诊的COPD急性加重期患者87例,根据随机数字表法分为对照组43例和治疗组44例,两组均给予止咳化痰平喘等基础治疗,给予机械通气。对照组给予阿奇霉素0.25 g口服,2次/d。治疗组在以上治疗基础上加服中药,分2次饭后服,1剂/d。两组均治疗1个月。比较两组疗效、机械通气前及通气48 h后pH值、PaO2和PaCO2。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果总有效率对照组74.42%,治疗组93.18%,比较差异有统计学意义(P<0.05)。通气48 h后对照组pH值、PaO2和PaCO2[7.31±0.08、(221.5±25.0)、(59.3±9.0)mm Hg(1 mm Hg=0.133 kPa)]与治疗组[7.40±0.11、(203.1±22.8)、(48.0±7.6)mm Hg]比较差异均有统计学意义(均P<0.05)。结论中药治疗COPD急性加重期可显著改善通气效果,整体效果满意,值得临床应用。
Objective To evaluate the effect of traditional Chinese medicine in the treatment of acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods 87 patients with exacerbation of COPD from January 2011 to January 2014 were divided into control group (n = 43) and treatment group (n = 44) according to the random number table method. Both groups were given basic treatment of cough and phlegm and asthma , Given mechanical ventilation. The control group given azithromycin 0.25 g orally, 2 times / d. On the basis of the above treatment, the treatment group added traditional Chinese medicine, divided into 2 meals, 1 dose / d. Both groups were treated for 1 month. The curative effect of the two groups were compared before the mechanical ventilation and 48 h after ventilation pH, PaO2 and PaCO2. Measurement data using t test, count data using χ2 test, P <0.05 for the difference was statistically significant. Results The total effective rate of 74.42% in the control group, 93.18% in the treatment group, the difference was statistically significant (P <0.05). The values of PaO2 and PaCO2 (7.31 ± 0.08, (221.5 ± 25.0) and (59.3 ± 9.0) mm Hg (1 mm Hg = 0.133 kPa)] in the control group after 48 h of ventilation were significantly higher than those in the treatment group [7.40 ± 0.11 and (203.1 ± 22.8), (48.0 ± 7.6) mm Hg] (all P <0.05). Conclusion Chinese medicine treatment of acute exacerbation of COPD can significantly improve the ventilation effect, the overall effect is satisfactory, worthy of clinical application.
