目的分析红细胞分布宽度与老年心力衰竭患者早期肾功能损伤的关系。方法选取2015年9月—2016年8月湖北医药学院附属人民医院收治的老年心力衰竭患者90例作为观察组,同期在本院体检的健康老年人90例作为对照组。比较两组患者及不同心功能分级老年心力衰竭患者红细胞分布宽度、肌酐、β_2微球蛋白、胱抑素C及肾小球滤过率,并分析红细胞分布宽度与老年心力衰竭患者肾小球滤过率的关系。结果观察组患者红细胞分布宽度、肌酐、β_2微球蛋白、胱抑素C高于对照组,肾小球滤过率低于对照组(P<0.05)。不同心功能分级老年心力衰竭患者红细胞分布宽度、肌酐、β_2微球蛋白、胱抑素C及肾小球滤过率和肾功能指标比较,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,红细胞分布宽度与老年心力衰竭患者肾小球滤过率呈负相关(r=-3.418,P=0.022)。多元线性回归分析结果显示,红细胞分布宽度是老年心力衰竭患者肾小球滤过率的影响因素[OR=1.759,95%CI(1.102,2.384),P=0.017]。结论红细胞分布宽度是老年心力衰竭患者肾小球滤过率的影响因素,可作为评估老年心力衰竭患者早期肾功能的指标。
Objective To analyze the relationship between the width of erythrocyte distribution and early renal function impairment in elderly patients with heart failure. Methods 90 elderly patients with heart failure admitted to the People’s Hospital of Hubei Medical College from September 2015 to August 2016 were selected as the observation group and 90 healthy elderly people were examined in our hospital at the same period as the control group. The distribution of erythrocyte width, creatinine, β_2 microglobulin, cystatin C and glomerular filtration rate were compared between two groups of patients and in elderly patients with different grades of heart failure. Pearson correlation analysis showed that the width of erythrocyte distribution was negatively correlated with glomerular filtration rate in elderly patients with heart failure (r = -3.418, P = 0.022). Multivariate linear regression analysis showed that the width of erythrocyte distribution was the influencing factor of glomerular filtration rate in elderly patients with heart failure [OR = 1.759, 95% CI (1.102, 2.344), P = 0.017]. Conclusion The width of erythrocyte distribution is the influencing factor of glomerular filtration rate in elderly patients with heart failure, which can be used as an index to evaluate the early renal function in elderly patients with heart failure.
