目的 :探讨定制人工膝关节假体在膝关节周围恶性骨肿瘤保肢治疗中的临床效果。方法 :对19例股骨远端或胫骨近端恶性骨肿瘤行瘤段骨广泛切除、定制人工膝关节假体置换术。结果 :随访 1年 5个月～ 5年 ,膝关节活动度为伸 0° ,屈 90°～ 10 5°。 19例患者中发生假体周围积液和行走后踝关节痛各 1例 ,无感染、假体松动及断裂 ,无肿瘤复发。 1例发生肺转移 ,带瘤存活 ,2例因肺及其他部位转移死亡。结论 :定制人工膝关节假体治疗膝关节周围恶性骨肿瘤能尽早及较好地恢复膝关节功能 ,尤其适合于骨破坏严重 ,骨支架作用基本丧失者。
Objective : To explore the clinical effect of custom artificial knee prosthesis in the limb salvage treatment of malignant bone tumors around the knee. Methods: A total of 19 cases of malignant bone tumors of the distal femur or proximal tibia were treated with extensive resection of the tumor and custom artificial knee prosthesis replacement. Results: The follow-up period was 1 year 5 months to 5 years. The degree of motion of the knee joint was 0°, 90° to 105°. In 19 patients, there was one case of peri-implant effusion and one case of ankle pain after walking, no infection, prosthesis loosening and fracture, and no tumor recurrence. One patient developed lung metastasis with tumor survival, and two patients died of metastasis due to lung and other sites. Conclusion : Customized artificial knee prosthesis for the treatment of malignant bone tumors around the knee joint can restore knee function as soon as possible and better, especially suitable for those with severe bone destruction and basic bone loss.
