目的探讨64排螺旋CT头颈部CT血管成像(CTA)技术的应用。方法使用GE64排Light Speed VCT螺旋CT机对35例缺血性脑卒中和短暂性脑缺血发作患者行头颈部CTA扫描,使用欧苏碘海醇造影剂20 ml,盐水20 ml预扫描,先行小剂量对比剂测试颈动脉时间-密度曲线,设定最佳延迟时间,第1组平扫时间与第2组增强扫描延迟时间之和为0.5的整倍数。将增强前后数字化图像进行减影,去除骨结构,从减影图像中进行血管三维重建。结果35例中34例CT血管减影成像均能良好显示颈内动脉、椎动脉及其主要分支,无骨质干扰;1例因头部轻微运动,颈椎骨质未能完全减去。结论头颈部CTA扫描应用减影成像技术,去除骨结构对血管的影响,在工作站后处理图像上可更清楚地显示头颈部血管,为诊断脑血管病变提供丰富图像。
Objective To explore the application of 64-slice spiral CT head-neck CT angiography (CTA) technique. Methods 35 patients with ischemic stroke and transient ischemic attack underwent GE CTA scan with a Light Speed VCT CT scanner. The CTA scan was performed with 20 ml of OSOI and 20 ml of saline. Low-dose contrast agent test carotid artery time-density curve, set the optimal delay time, the first group of scan time and the second group of enhanced scan delay time and the integral multiple of 0.5. The enhanced digital image before and after subtraction, removal of bone structure, three-dimensional reconstruction of blood vessels from subtraction images. Results 34 cases of CT angiography in 35 cases showed good performance of internal carotid artery, vertebral artery and its major branches without bone interference; 1 case of slight movement of the head, cervical bone quality can not be completely subtracted. Conclusion CTA scan of head and neck using subtraction imaging technology to remove the impact of bone structure on the blood vessels in the workstation post-processing images can be more clearly show the head and neck blood vessels for the diagnosis of cerebrovascular disease provides rich images.
