尿中白蛋白的轻度增加是胰岛素依赖型糖尿病(IDDM)早期肾病变的肾小球损害的先兆。最近发现,一些具有或不具有微量白蛋白尿的IDDM病人尿维生素A醇结合蛋白(RBP)排泄量增加,而RBP是肾小管蛋白尿或低分子量(LMW)蛋白尿的标志。作者采用RBP作为近曲肾小管回吸收LMW蛋白受损的指标,研究具有微量白蛋白尿和肾小管LMW蛋白尿的胰岛素依赖型糖尿病患者的高血压发病率。
A slight increase in urinary albumin is a precursor to glomerular damage in early diabetic nephropathy with insulin-dependent diabetes mellitus (IDDM). Recently, it has been found that some patients with IDDM with or without microalbuminuria have increased excretion of urinary vitamin A alcohol binding protein (RBP), which is a hallmark of renal tubular proteinuria or low molecular weight (LMW) proteinuria. The authors used RBP as an indicator of proximal tubular reabsorption of LMW protein to study the incidence of hypertension in insulin-dependent diabetes mellitus patients with microalbuminuria and renal tubular LMW proteinuria.
