目的观察分析甘精胰岛素联合瑞格列奈在2型糖尿病慢性肾功能不全患者中的降糖作用。方法 60例2型糖尿病慢性肾功能不全患者,根据不同治疗方式将患者分为观察组和对照组,各30例。对照组给予皮下注射胰岛素治疗,观察组患者给予甘精胰岛素联合瑞格列奈治疗,观察比较两组患者的疗效。结果对照组空腹血糖(FPG)为(11.22±1.17)mmol/L,餐后2 h血糖(2 h PG)为(10.55±3.15)mmol/L,糖化血红蛋白(Hb A1c)为(7.93±1.37)%;观察组FPG为(8.34±1.35)mmol/L,2 h PG为(7.30±1.82)mmol/L,Hb Alc为(6.23±1.13)%,观察组三项血糖指标均优于对照组,差异均具有统计学意义(P<0.05)。对照组24 h尿白蛋白排泄率为(1.58±0.36)mmol/L,血尿素氮(BUN)为(11.58±5.65)mmol/L,血清肌酐(SCr)为(279.96±129.87)μmol/L;观察组24 h尿白蛋白排泄率为(1.14±0.33)mmol/L,BUN为(9.54±5.20)mmol/L,SCr为(177.76±121.33)μmol/L,观察组三项指标明显优于对照组,差异具有统计学意义(P<0.05)。结论甘精胰岛素联合瑞格列奈应用于2型糖尿病慢性肾功能不全患者的治疗具有良好的降糖作用,还有利于肾功能的改善及恢复,治疗效果好,值得推广。
Objective To observe the hypoglycemic effect of glargine and repaglinide in patients with type 2 diabetes mellitus and chronic renal failure. Methods Sixty patients with type 2 diabetes mellitus with chronic renal insufficiency were divided into observation group and control group according to different treatment methods, 30 cases in each group. The control group was given subcutaneous insulin therapy, and the observation group was treated with glargine and repaglinide. The curative effect of the two groups was observed and compared. Results Fasting plasma glucose (FPG) was (11.22 ± 1.17) mmol / L in control group, (10.55 ± 3.15) mmol / L in 2 h postprandial blood glucose and 7.93 ± 1.37 in Hb A1c group %. The FPG in observation group was (8.34 ± 1.35) mmol / L, PG in 2 h was (7.30 ± 1.82) mmol / L and Hb Alc was (6.23 ± 1.13)% in control group. The differences were statistically significant (P <0.05). The urinary albumin excretion rate in the control group was (1.58 ± 0.36) mmol / L at 24 hours, (11.58 ± 5.65) mmol / L and (279.96 ± 129.87) μmol / L respectively; The urinary albumin excretion rate in the observation group was (1.14 ± 0.33) mmol / L, (9.54 ± 5.20) mmol / L, and SCr was (177.76 ± 121.33) μmol / L, Group, the difference was statistically significant (P <0.05). Conclusion The application of glargine and repaglinide in patients with type 2 diabetes mellitus with chronic renal insufficiency has a good hypoglycemic effect, which is also beneficial to the improvement and recovery of renal function. The treatment effect is worthy of promotion.
