目的 评价门静脉螺旋CTAP(S- CTAP) 三维血管成像的临床价值,并比较两种三维成像方法:表面阴影显示(SSD) 和最大强度投影(MIP) 对门静脉显示的差别。方法 对36 例肝癌患者经肠系膜上动脉(SMA) 置管行S- CTAP。结果 ①门静脉S- CTAP三维血管成像能清楚显示门脉4 ~5 级小分支,并显示门脉分支受压、推移、破坏、中断等多种形态改变,优于间接门脉DSA。②SSD 能良好显示门脉主干及1 ~2 级大分支的立体解剖关系,MIP则对门脉4 ~5 级小分支和因病变引起的形态改变显示较好。结论 ①S- CTAP三维成像门脉显示优于间接门脉DSA。②SSD 能显示门脉血管树的立体观,而MIP显示门脉的小分支较佳
Objective To evaluate the clinical value of three-dimensional portal angiography in the diagnosis of portal venous helical CTAP (S-CTAP) and to compare the two methods of three-dimensional imaging: the difference between the display of portal surface (SSD) and maximum intensity projection (MIP) Methods Thirty-six HCC patients were treated with S-CTAP through the superior mesenteric artery (SMA). Results ① S-CTAP three-dimensional portal angiography of portal vein can clearly show the small branches of the fourth to fifth branches of the portal vein and show various morphological changes such as compression, displacement, destruction and interruption of portal branches, which is better than indirect portal vein DSA. ② SDSD can well show the main anatomy of the portal vein and the major branches of the first to the second grade. The MIP shows good performance on the minor branches of the portal vein from grade 4 to 5 and the morphological changes caused by the lesions. Conclusion ① S-CTAP three-dimensional imaging portal vein display superior to indirect portal DSA. ② SDSD can show the three-dimensional view of the portal vein tree, and MIP shows a small branch of the portal vein better
