目的:探讨17β羟化类固醇脱氢酶(17β-HSD)5在乳腺癌和癌旁非恶性乳腺组织中的表达及其与患者临床特征之间的关系。方法:该实验选取2004年1月~2007年5月进行乳腺癌手术病人的乳腺癌组织及癌旁非恶性乳腺组织76例,全部为女性。运用免疫细胞化学的方法测定17β-HSD 5在乳腺癌及同一患者癌旁非恶性乳腺组织中的表达,并采用统计学方法探讨该酶的表达与临床资料的关系。结果:17β-HSD 5在乳腺癌组织中表达阳性率为44%,在癌旁组织中表达的阳性率为74%,二者差异有统计学意义(χ2=7.758 6,P=0.005 3)。癌旁组织中17β-HSD 5表达与患者年龄(r=0.347 63,P=0.0134)、月经状态(r=0.398 59,P=0.004 6)、ER(χ2=9.083 5,P=0.0026)的表达呈明显正相关,与癌旁组织中PR(χ2=7.487 1,P=0.006 2)呈负相关。在乳腺癌组织中,没有发现该酶与患者年龄、月经状态、ER、PR、AR、P185、肿瘤分期、肿物大小以及阳性淋巴结数量之间的相关性。结论:17β-HSD 5在癌旁组织中阳性率明显高于癌组织中的阳性率,并且AR在癌旁组织与癌组织也有同样的差异,表明17β-HSD 5在乳腺癌的发生发展中起作用,也间接说明了雄激素通过AR而发挥了在乳腺组织中的保护作用。绝经后妇女17β-HSD 5表达明显高于绝经前妇女,这也反应绝经后妇女局部组织中雄激素代谢的改变。
Objective: To investigate the expression of 17β-hydroxysteroid dehydrogenase (17β-HSD) 5 in breast cancer and adjacent non-malignant breast tissue and its relationship with clinical features. Methods: This experiment selected 76 cases of breast cancer tissues and adjacent non-malignant breast tissue from January 2004 to May 2007 for breast cancer patients, all of whom were female. Immunocytochemistry was used to determine the expression of 17β-HSD 5 in breast cancer and adjacent non-malignant breast tissues. The relationship between the expression of 17β-HSD 5 and clinical data was also investigated by statistical methods. Results: The positive rate of 17β-HSD 5 in breast cancer tissues was 44%, while it was 74% in paracancerous tissues. The difference was statistically significant (χ2 = 7.758 6, P = 0.005 3). The expression of 17β-HSD 5 in paracancerous tissues was significantly correlated with age (r = 0.347 63, P = 0.0134), menstrual status (r = 0.398 59, P = 0.004 6) There was a significant positive correlation between PR (χ2 = 7.487 1, P = 0.006 2) and adjacent tissues. In breast cancer tissues, no correlation was found between the enzyme and patient age, menstrual status, ER, PR, AR, P185, tumor stage, tumor size, and number of positive lymph nodes. Conclusion: The positive rate of 17β-HSD 5 in paracancer tissues is significantly higher than that in cancerous tissues, and the same difference between AR and paracancerous tissues is also shown in cancer tissues, indicating that 17β-HSD 5 plays an important role in the development of breast cancer Role, but also indirectly shows that androgen through AR and played a protective role in breast tissue. Postmenopausal women 17β-HSD 5 expression was significantly higher than pre-menopausal women, which also reflects changes in androgen metabolism in the local tissue of postmenopausal women.
