目的:探讨Oct-4蛋白在甲状腺腺瘤、甲状腺乳头状癌、甲状腺滤泡癌、甲状腺髓样癌中的表达及意义。方法:运用免疫荧光法检测15例甲状腺腺瘤,30例甲状腺乳头状癌,2例甲状腺滤泡状癌,3例髓样癌中Oct-4的表达。结果:Oct-4在甲状腺腺瘤、甲状腺乳头状癌、甲状腺滤泡癌和甲状腺髓样癌中均有表达。与甲状腺腺瘤相比,Oct-4在甲状腺乳头状癌中表达率明显增高,其表达与患者的年龄、性别、肿块位置、肿块大小和有无转移无明显相关性。Oct-4在甲状腺髓样癌和滤泡癌中表达水平最高。结论:①Oct-4阳性细胞可能是甲状腺组织中存在的干细胞,可能与甲状腺肿瘤的形成有关。②与良性甲状腺肿瘤相比,甲状腺癌中Oct-4表达水平明显增高。③甲状腺髓样癌及滤泡癌中残留Oct-4阳性细胞较多,与肿瘤恶性程度较高的特征一致。
Objective: To investigate the expression and significance of Oct-4 protein in thyroid adenoma, papillary thyroid carcinoma, follicular thyroid carcinoma and medullary thyroid carcinoma. Methods: The expression of Oct-4 in 15 cases of thyroid adenoma, 30 cases of thyroid papillary carcinoma, 2 cases of thyroid follicular carcinoma and 3 cases of medullary carcinoma were detected by immunofluorescence. Results: Oct-4 was expressed in thyroid adenoma, thyroid papillary thyroid carcinoma, thyroid follicular carcinoma and medullary thyroid carcinoma. Compared with thyroid adenoma, the expression of Oct-4 in papillary thyroid carcinoma was significantly higher than that of thyroid adenoma. There was no significant correlation between the expression of Oct-4 and age, sex, tumor location, tumor size and metastasis. Oct-4 is most abundantly expressed in medullary and follicular carcinomas. Conclusion: (1) Oct-4 positive cells may be stem cells in thyroid tissue, which may be related to the formation of thyroid tumors. ② Compared with benign thyroid tumors, thyroid cancer Oct-4 expression was significantly higher. ③ There were more Oct-4 positive cells in thyroid medullary carcinoma and follicular carcinoma, which was consistent with the higher degree of tumor malignancy.
