目的探讨抗角蛋白抗体(AKA)对于早期类风湿关节炎(RA)的诊断价值。方法对68例早期RA患者(RA组)、90例非RA性自身免疫性疾病患者(非RA组)和50例正常对照者的血清AKA和RF进行了测定比较。结果在RA患者中,AKA的阳性率为58.8%,显著高于其在非RA组和正常对照中的阳性率(P<0.01)。对于诊断RA,AKA的敏感性和特异性分别为70.8%和96.6%;RF的敏感性和特异性分别为73.9%和83.8%。AKA和RF的特异性相比较,差异有统计学意义(P<0.05)。结论 AKA是RA的特异性血清指标,对RA的早期诊断、早期治疗及预后判断具有一定实用价值。
Objective To investigate the diagnostic value of anti-keratin antibody (AKA) for early rheumatoid arthritis (RA). Methods Serum AKA and RF of 68 patients with early RA (RA group), 90 patients with non-RA autoimmune disease (non-RA group) and 50 normal controls were compared. Results The positive rate of AKA in RA patients was 58.8%, which was significantly higher than that in non-RA patients and normal controls (P <0.01). The sensitivity and specificity of AKA for diagnosis of RA were 70.8% and 96.6%, respectively; the sensitivity and specificity of RF were 73.9% and 83.8% respectively. AKA and RF specificity compared to the difference was statistically significant (P <0.05). Conclusions AKA is a specific serum marker of RA, which has some practical value in the early diagnosis, early treatment and prognosis of RA.
