目的探讨胃癌病人穿孔素和颗粒酶的表达情况及其临床意义。方法泸州医学院附属医院普通外科2008-2009年采用免疫组织化学染色PV法,检测穿孔素(PFP)和颗粒酶(GrB)在胃切缘组织、癌旁组织、胃癌组织及相应转移淋巴组织中的表达情况,并结合临床、病理特征情况进行分析。结果 (1)GrB在切缘组织-癌旁组织-胃癌组织中阳性表达率的差异无统计学意义(P>0.05);(2)GrB(+)与GrB(-)胃癌组织所对应同一病人的胃切缘组织、癌旁组织GrB表达的差异有统计学意义(P<0.01);(3)以PFP细胞作为阳性对照,无论GrB表达与否,发现PFP蛋白在切缘组织、癌旁组织、胃癌组织胃上皮细胞的胞浆中均未见表达;(4)GrB在胃癌原发灶和淋巴结转移灶阳性表达的差异无统计学意义(P=0.130)。PFP在所有淋巴结转移癌细胞中为阴性表达;(5)GrB在胃癌组织的表达情况结合相应临床病理特征分析发现,GrB表达的原发肿瘤可能对肿瘤细胞的淋巴结转移起到抑制作用。结论 GrB(+)胃癌细胞与PFP非共表达,未能证实GrB(+)胃癌细胞具有淋巴样细胞分化表型。GrB可能参与胃癌发生的全过程,是胃癌形成的早期事件。
Objective To investigate the expression of perforin and granzyme in patients with gastric cancer and its clinical significance. Methods The general surgery department of Affiliated Hospital of Luzhou Medical College from 2008 to 2009 was immunohistochemically stained with PV method to detect the expression of perforin (PFP) and granzyme (GrB) in gastric marginal tissues, paracancerous tissues, gastric cancer tissues and corresponding metastatic lymphoid tissues Of the expression, combined with clinical, pathological features of the situation analysis. Results (1) There was no significant difference in the positive expression rate of GrB in the marginal tissue - adjacent tissue - gastric cancer (P> 0.05). (2) The same patient with GrB (-) (P <0.01). (3) PFP cells were used as positive control. No expression of GrB was found in the marginal tissues and paracancerous tissues (P <0.05). (4) There was no significant difference in the positive expression of GrB in the primary tumor and lymph node metastasis of gastric cancer (P = 0.130). PFP was negatively expressed in all lymph node metastatic cancer cells. (5) The expression of GrB in gastric cancer combined with the corresponding clinicopathological features showed that GrB-expressing primary tumor may inhibit the lymph node metastasis of tumor cells. Conclusion The co-expression of Grb (+) gastric cancer cells and PFP failed to confirm the lymphoid differentiation phenotype of GrB (+) gastric cancer cells. GrB may participate in the whole process of gastric cancer and is an early event of gastric cancer.
