本文对体表信号平均心电图对心肌梗塞(MI)病人自发或心室电程序刺激诱发单形性持续性室性心动过速(SMVT)的预测作用进行了研究,研究对象包括44例正常对照(Ⅰ组)、31例无SMVT的MI病人(Ⅱ组)和17例有SMVT的MI病人(Ⅲ组).结果为:Ⅲ组心室晚电位检出率明显高于Ⅰ组和Ⅱ组,在Ⅲ组与Ⅰ组、Ⅲ组与Ⅱ组间,不仅常用的时域指标TQRS、D_(40)、L_(40)差异显著,且DQRS、RQRS、LD和F差异也具有显著性;尤其F尚可明确区分Ⅰ组与Ⅱ组.提示:①DQRS、RQRS和LD可能更好地反映常规采用的时域指标TQRS、D_(40)和L_(40)中异常成份的作用,②指标F可反映出心肌损害所致电活动的碎裂特性,③与TQRS,D_(40)和L_(40)相比,在预测MI病人发生SMVT方面,F和LD可分别提高其预测敏感性和特异性.
In this paper, we investigated the predictive value of body surface signal average electrocardiogram (ECG) on the monomorphic continuous ventricular tachycardia (SMVT) induced by spontaneous or ventricular electrical stimulation in patients with myocardial infarction (MI). The subjects included 44 normal controls 31 patients with MI without SMVT (group Ⅱ) and 17 patients with MI with SMVT (group Ⅲ) .The results showed that the detection rate of ventricular late potential in group Ⅲ was significantly higher than that in group Ⅰ and Ⅱ, and in group Ⅲ Compared with group Ⅰ, group Ⅲ and group Ⅱ, not only the commonly used time domain indexes TQRS, D40 and L40 were significantly different, but also the differences of DQRS, RQRS, LD and F were significant; especially F was still clear DQRS, RQRS and LD may reflect the function of TQRS, D_ (40) and L_ (40), which are commonly used time-domain indexes, and ②Function F may reflect myocardial damage (3) Compared with TQRS, D_ (40) and L_ (40), F and LD could improve their predictive sensitivity and specificity respectively in predicting the occurrence of SMVT in MI patients.
