目的 探讨非小细胞肺癌 (NSCLC)组织中基质金属蛋白酶 2 (MMP 2 )和基质金属蛋白酶 9(MMP 9)的表达水平及其预后意义。方法 采用免疫组化SP法检测 2 8例正常肺组织、3 2例癌旁异型增生肺组织和 62例NSCLC组织中MMP 2和MMP 9的表达水平。结果 NSCLC组织中MMP 2 ( 77.4% )和MMP 9( 80 .6% )阳性表达率均显著高于癌旁异型增生组织和正常肺组织 (P均 <0 .0 0 5 )。腺癌和腺鳞癌中MMP 2和MMP 9阳性表达率均显著高于鳞癌 (P均 <0 .0 5 )。MMP 2和MMP 9的阳性表达率随TNM分期而增加 ,Ⅲ +Ⅳ期显著高于Ⅰ +Ⅱ期 (P <0 .0 2 5 ,P <0 .0 5 )。有淋巴结转移者MMP 2和MMP 9阳性表达率高于无淋巴结转移者 (P <0 .0 0 5 ,P <0 .0 5 )。生存 1、3年的患者MMP 2和MMP 9阳性表达率均高于 5年以上存活者 (P <0 .0 5 ,P <0 .0 0 5 )。结论 NSCLC组织内MMP 2和MMP 9阳性表达可能是预测肿瘤恶性行为的有用指标。
Objective To investigate the expression of matrix metalloproteinase 2 (MMP 2) and matrix metalloproteinase 9 (MMP 9) in non-small cell lung cancer (NSCLC) and its prognostic significance. Methods Immunohistochemical SP method was used to detect the expression of MMP 2 and MMP 9 in 28 normal lung tissues, 32 paracancerous hyperplastic lung tissues and 62 NSCLC tissues. Results The positive rates of MMP 2 (77.4%) and MMP 9 (80.6%) in NSCLC tissues were significantly higher than those in para-cancerous hyperplastic tissues and normal lung tissues (P < 0.05). The positive expression rates of MMP 2 and MMP 9 in adenocarcinoma and adenosquamous carcinoma were significantly higher than those in squamous carcinoma (P < 0.05). The positive expression rates of MMP 2 and MMP 9 increased with TNM stage, and the levels of III + IV were significantly higher than those of stage I + II (P <0. 025, P <0. 05). The positive expression rates of MMP 2 and MMP 9 in patients with lymph node metastasis were higher than those without lymph node metastasis (P <0.05, P <0.05). The positive expression rates of MMP 2 and MMP 9 in patients who survived for 1 or 3 years were higher than those surviving for more than 5 years (P <0.05, P <0.05). Conclusion The positive expression of MMP 2 and MMP 9 in NSCLC may be a useful index to predict the malignant behavior of tumors.
