目的 提高鞍结节脑膜瘤的显微外科治疗水平。方法 报告 6 5例鞍结节脑膜瘤的临床表现 ,诊断和显微外科治疗结果。结果 所有病例均在手术显微镜下进行 ,肿瘤全切除率为93 8% ,手术死亡率为 3 1%。术中一侧或双侧嗅神经的保留率为 86 %。仔细辩认和分离垂体柄 ,防止损伤后发生尿崩症 ,垂体柄的保存率达 95 4%。术中颈内动脉或其它血管破裂出血 ,可行显微缝合。大多数病人术后视力得到满意的恢复。结论 根据肿瘤的部位和大小 ,选择恰当的手术入路。应用显微外科技术能够显著提高肿瘤的全切率和降低手术死亡率。
Objective To improve the microsurgical treatment of saddle nodular meningioma. Methods Sixty-five patients with saddle nodular meningiomas were reported for clinical presentation, diagnosis and microsurgical treatment. Results All the cases were performed under a surgical microscope. The total tumor removal rate was 93.8% and the operative mortality rate was 31%. Intraoperative or bilateral olfactory nerve retention rate of 86%. Carefully identify and separate pituitary stalk to prevent diabetes insipidus occurs after injury, pituitary stalk preservation rate of 95 4%. Intraoperative carotid artery or other blood vessels rupture bleeding, viable microscopic suture. Most patients have a satisfactory recovery of visual acuity. Conclusion According to the tumor site and size, choose the appropriate surgical approach. Application of microsurgical techniques can significantly improve the rate of tumor resection and reduce operative mortality.
