目的探讨微小染色体维持蛋白-5(MCM5)与P16~(INK4A)在HPV16感染的宫颈鳞癌中表达及意义。方法采取实时荧光定量PCR及免疫组化,测定存在HPV16感染的正常宫颈、CIN 1、CIN 2/3及宫颈鳞癌组织中MCM5、P16~(INK4A)的mRNA、蛋白表达情况。结果 (1)MCM5 mRNA及蛋白在HPV16感染的不同程度宫颈病变组织中表达趋势一致,随CIN程度的增加表达增高。MCM5 mRNA及蛋白在鳞癌中表达显著较各级别上皮内病变及正常组织高(P<0.01)。MCM5蛋白的表达和年龄无关联(P>0.05);与病理分级、临床分期相关(P<0.01);(2)不同程度宫颈病变中P16~(INK4A)mRNA、蛋白表达趋势一致,随CIN程度增加表达增高。P16~(INK4A) mRNA及蛋白在鳞癌中的表达明显高于正常及CIN 1组织(P<0.01);和CIN2/3之间差异无统计学意义(P>0.05)。P16~(INK4A)蛋白在宫颈癌中的表达和患者年龄及临床分期没有关联(P>0.05);和病理分级有关(P<0.01);(3)MCM5和P16~(INK4A)蛋白表达呈正相关(r=0.497,P<0.01)。结论宫颈鳞癌中MCM5和P16~(INK4A)存在高表达,可能与其发生、进展相关。MCM5可较好的反映宫颈的恶性增生,与P16~(INK4A)联合检测对于完善CIN分级及预后的判断具有一定的参考价值。
Objective To investigate the expression and significance of minichromosome maintenance protein-5 (MCM5) and P16 INK4A in cervical squamous cell carcinoma (HPV16) infected with HPV16. Methods The mRNA and protein expressions of MCM5 and P16 INK4A in normal cervical tissue, CIN 1, CIN 2/3 and cervical squamous cell carcinoma were detected by real-time fluorescence quantitative PCR and immunohistochemistry. Results (1) The expression of MCM5 mRNA and protein in cervical lesions with different degrees of HPV16 infection was consistent, with the increase of CIN expression. The expression of MCM5 mRNA and protein in squamous cell carcinoma was significantly higher than that in all epithelial lesions and normal tissues (P <0.01). (P <0.01); (2) The expression of P16 INK4A mRNA and protein in different degree of cervical lesions had the same trend, with the degree of CIN Increased expression increased. The expression of P16 INK4A mRNA and protein in squamous cell carcinoma was significantly higher than that in normal and CIN 1 tissues (P <0.01). There was no significant difference between P16 INK4A and CIN2 / 3 (P> 0.05). There was no correlation between the expression of P16 INK4A protein in cervical cancer and clinical stage (P> 0.05), and the pathological grade (P <0.01). (3) There was a positive correlation between the expression of P16 INK4A protein and cervical intraepithelial neoplasia (r = 0.497, P <0.01). Conclusion There is a high expression of MCM5 and P16 INK4A in cervical squamous cell carcinoma, which may be related to their occurrence and progression. MCM5 can better reflect the malignant cervical hyperplasia, and P16 ~ (INK4A) combined detection of CIN grade and improve the prognosis of judgment has some reference value.
