目的 研究牵引成骨技术在下颌骨功能性重建中的可行性及优势。方法 使用 3种牵引器对 8例由肿瘤或囊肿切除所致下颌骨缺损患者进行牵引成骨矫治。患者的平均年龄为31 5岁 (8- 5 4岁 )。其中 2例升支缺损患者使用特制牵引器以保持升支正常高度 ;2例患者采用了特制的三焦点牵引器 ;3例患者使用了垂直牵引器。结果 7例患者的下颌骨缺损均按术前设计的方向及长度得以成功矫治。 1例患者远中段 2 3mm的缺损由于传送盘固位螺钉松动而导致牵引成骨失败。结论 牵引成骨技术可用于下颌骨缺损的重建修复。下颌骨缺损牵引成骨重建的最佳时机是肿瘤或囊肿切除术中。
Objective To study the feasibility and advantage of traction osteogenesis in mandibular functional reconstruction. Methods Three tractors were used to treat 8 cases of mandibular defects caused by tumor or cyst excision. The average age of patients was 31 5 years old (8- 5 4 years old). Two of the patients with ascending defect had a special tractor to maintain the ascending height of the ascending branch; two patients had a special triple-focus tractor; and three had a vertical tractor. Results The mandibular defects of 7 patients were successfully treated according to the preoperative design direction and length. 1 case of distal midline 2 3mm defect due to looseness of the tray fixation screw lead to traction osteoporosis failure. Conclusion Traction osteogenesis can be used to reconstruct and repair mandibular defects. Mandibular defect traction osteogenic reconstruction is the best time to tumor or cyst excision.
