目的:评价血清半胱氨酸蛋白酶抑制剂-C(Cys C)相关方程在慢性肾脏病(CKD)患者肾小球滤过率(GFR)计算中的临床应用价值。方法:选取179例慢性肾脏病患者,运用MDRD方程、MDRD简化方程、Hoek方程、2006年中华医学会肾脏病学分会推荐的两个方程计算GFR,与99mTc-DTPA肾动态显像测得的GFR相比较。结果:各方程估算的GFR和99mTc-DTPA肾动态显像测得的GFR均呈相关,其中2006年中华医学会推荐方程2计算的GFR与99mTc-DTPA肾动态显像测得的GFR相关性最好。Bland-Altman分析显示MDRD简化方程计算的GFR和99mTc-DTPA肾动态显像测得的GFR一致性最好。结论:在Cys C相关方程中,2006年中华医学会肾脏病分会推荐公式2计算的GFR,与99mTc-DTPA肾动态显像测得GFR相关性及一致性较高,在临床上可用来计算慢性肾脏病患者的GFR。
OBJECTIVE: To evaluate the clinical value of serum Cysteine Protease Inhibitor-C (Cys C) correlation equation in the calculation of glomerular filtration rate (GFR) in patients with chronic kidney disease (CKD). Methods: A total of 179 patients with chronic kidney disease were enrolled in this study. GFR was calculated by using MDRD equation, MDRD equation, Hoek equation and two equations recommended by Chinese Society of Nephrology in 2006. GFR was measured with 99mTc-DTPA renal dynamic imaging Compared. Results: GFR estimated by each equation was correlated with GFR measured by 99mTc-DTPA renal dynamic imaging. The correlation between GFR and 99mTc-DTPA renal dynamic imaging in GFR recommended by Chinese Medical Association in 2006 was the highest it is good. Bland-Altman analysis showed that GFR calculated by MDRD simplified equation and GFR99mTc-DTPA renal dynamic imaging best GFR agreement. Conclusion: In the Cys C correlation equation, GFR calculated by the Chinese Society of Nephrology Branch of the Chinese Society of Diseases recommended by Formula 2 in 2006 is highly correlated with GFR measured by 99mTc-DTPA renal dynamic imaging and can be used clinically to calculate chronic GFR in patients with kidney disease.
