目的探索经股动静脉体外循环能否有效提高心脏骤停犬的循环复苏率,缩短自主循环恢复时间及保持复苏后循环平稳。方法16只犬随机分2组,用窒息方法造成动物心脏骤停,10分钟后开始复苏。A组用常规复苏技术,即人工呼吸、胸外心脏按压、静注肾上腺素。B组在A组基础上加用经股动静脉闭胸式体外循环。观察动物循环复苏情况。结果A组仅1只犬在复苏后14分钟恢复自主循环(1/8)。B组8只犬全部复苏成功(8/8),自主循环恢复时间408±23分钟。B组复苏时心脏灌注压明显高于A组(P<001)。辅助循环5~10分钟后停止,自主循环能保持相对平稳。结论闭胸式体外循环可有效地提高实验犬的心脏灌注压及循环复苏成功率。
Objective To explore whether the transthoracic and extrahepatic cardiopulmonary bypass can effectively improve the rate of circulatory revascularization in dogs with cardiac arrest, shorten the recovery time of spontaneous circulation and maintain the circulation stable after resuscitation. Methods Sixteen dogs were randomly divided into two groups. Animals were arrested by asphyxial method and resuscitated after 10 minutes. A group with conventional resuscitation techniques, that is, artificial respiration, chest compression, intravenous injection of epinephrine. Group B was added on the basis of group A by closed femoral vein and closed chest cardiopulmonary bypass. Observation of animal circulatory recovery. Results Only 1 dog in group A regained spontaneous circulation (1/8) 14 minutes after resuscitation. All 8 dogs in group B recovered successfully (8/8), and the recovery time of spontaneous circulation was 408 ± 23 minutes. Cardiac perfusion pressure was significantly higher in group B than in group A (P <001). Auxiliary cycle 5 to 10 minutes after the stop, self-cycle can be relatively stable. Conclusion Closed chest cardiopulmonary bypass can effectively improve the perfusion pressure of dogs and the success rate of circulatory resuscitation.
