目的:评价严重烧伤患者全身炎症反应综合征(SIRS)以及诊断SIRS的4 个要素对器官功能不全的影响。方法:对严重烧伤患者诊断SIRS的4 个要素进行评分,并与器官功能评分结果进行逐步多元回归分析。对不同诊断标准的SIRS发生时间与器官功能评分进行相关分析。结果:诊断SIRS的4 个要素中,对器官功能不全的影响从大到小依次是呼吸频率、心率、体温和白细胞计数。严重烧伤患者不同诊断标准的SIRS发生时间均与器官功能不全的发生显著相关。尤其以满足3个要素的发生时间与器官功能不全评分的相关系数最高。结论:诊断SIRS的不同要素对严重烧伤患者预后的判断有不同价值,严重烧伤患者SIRS的持续时间以及严重程度影响器官功能不全的发生。
Objective: To evaluate the impact of systemic inflammatory response syndrome (SIRS) and the diagnosis of SIRS in severely burned patients on organ dysfunction. Methods: Four factors of SIRS in patients with severe burn were scored, and multiple regression analysis was performed with the results of organ function score. Correlation analysis was made between SIRS occurrence time and organ function score of different diagnostic criteria. Results: The four factors of diagnosis of SIRS, organ dysfunction in descending order of respiratory rate, heart rate, body temperature and white blood cell count. The incidence of SIRS at different diagnostic criteria in patients with severe burns was significantly associated with the occurrence of organ dysfunction. In particular, to meet the three elements of the time of occurrence and organ dysfunction score the highest correlation coefficient. CONCLUSIONS: Different factors in the diagnosis of SIRS have different values in judging the prognosis of patients with severe burns. The duration and severity of SIRS in patients with severe burns affect the occurrence of organ dysfunction.
