目的分析探讨甲状腺微小癌(Thyroid microcarcinoma TMC)的超声表现特征及诊断价值,以期提高术前超声诊断的正确率及鉴别诊断水平。方法回顾分析我院2009至今门诊及住院经手术和病理确诊的83例甲状腺癌患者共108个病灶,分析癌结节的超声显像特征,包括结节的大小、形态、边界、内部回声、钙化灶及血流分布情况及颈部淋巴结转移情况。结果甲状腺微小癌声像图表征为低回声结节,实质回声不均匀,形态不规则72个,占86.7%;纵横比≥1共69个结节,占83.1%;边界不清晰或部分边界不清晰72个,占86.7%;尚清晰16个,占19.3%;内部可见微小钙化者42个,占50.6%;内部血流丰富42个,占50.6%。综上得之,边界不清晰或部分不清晰,实质性低回声,形态不规则,内部伴微小钙化同时伴有血流信号丰富是甲状腺微小癌的超声主要表现特点。结论甲状腺微小癌超声征象具有明显的特征性指标,超声影像检查对甲状腺微小癌是一种有效的诊断方法,能够早期发现和诊断甲状腺微小癌,从而提高术前甲状腺微小癌的诊断率,为临床治疗提供有效的信息。
Objective To investigate the characteristics and diagnostic value of ultrasonography of Thyroid microcarcinoma (TMC) in order to improve the accuracy and differential diagnosis of preoperative ultrasound. Methods A total of 108 lesions of 83 patients with thyroid cancer diagnosed by surgery and pathology in our hospital from 2009 to present were retrospectively analyzed. The features of nodules were analyzed including the size, shape, boundary, internal echo, calcification Foci and blood flow distribution and cervical lymph node metastasis. Results The thyroid microcirculation was characterized by hypoechoic nodules with non-uniform echoes, with 72 irregular or irregular patterns, accounting for 86.7%. The aspect ratio ≥1 consisted of 69 nodules, accounting for 83.1%. The border was not clear or part of the border was not Clear 72, accounting for 86.7%; still clear 16, accounting for 19.3%; internal micro-calcification were 42, accounting for 50.6%; internal blood flow-rich 42, accounting for 50.6%. To sum up, the border is not clear or partially unclear, substantial hypoechoic, irregular shape, accompanied by micro-calcification of the internal blood flow signal-rich thyroid cancer is the main characteristics of micro-ultrasound. Conclusion Ultrasonographic features of thyroid microcarcinoma have obvious characteristic indexes. Ultrasonography is an effective diagnostic method for thyroid microcarcinoma, which can detect and diagnose thyroid microcarcinoma early, so as to improve the diagnostic rate of preoperative thyroid microcarcinoma. Treatment provides effective information.
